Serum Endocan Level And The Relationship With Blood Pressure In A Community Middle-aged And Elderly Population

Objectives The aim of this study was to investigate the level of serum endocan in a community middle-aged and elderly population,and to analyze the relationship between serum endocan level and cardiovascular risk factors such as blood pressure,blood glucose,blood lipids and carotid intima-media thickness(CIMT).Methods A total of 489 subjects aged over 40 years old in Zhangzhou City were included in the cross-sectional survey from May 2016 to May 2017.The basic data,physical examination,laboratory examination and carotid ultrasound examination were conducted.The serum endocan level was determined by ELISA.The independent sample t-test,nonparametric test,chi-square test were used to compare general clinical characteristics of different serum endocan levels.Stratified analysis of blood pressure was applied to explore the change of serum endocan level in different blood pressure levels.The binary logistic regression analysis was used to analyze the relationship of serum endocan level with cardiovascular risk factors.Results 1.A total of 489 subjects were enrolled in this study,including 199 men and 20 women,with the median age of 51.0(46.0,59.0)and the sex ratio of 1:1.5.The average serum endocan level was 151.39(102.84,215.32)pg/ml.Grouping serum endocan by ROC curve cut-off point,there was no difference in age and gender between high and low level endocan groups.The diastolic pressure,fasting blood glucose and the prevalence of hypertension in the low serum endocan group were higher than that in the high serum endocan group(P<0.05).The waist circumference and CIMT were higher in the low serum endocan group,although there were no statistical significance,but the P value was close to 0.05.No significant differences were observed between the two groups regarding the age,gender,body mass index,systolic pressure,glycosylated hemoglobin,blood lipids,smoking,the prevalence of diabetes and dyslipidemia,alcohol consumption and lack of exercise.2.Serum endocan level was negatively correlated with waist circumference(P<0.05),and was not significantly correlated with blood pressure,blood lipids,blood glucose,uric acid,and estimated glomerular filtration rate.3.The level of serum endocan was higher in non-hypertensive group compared with hypertensive group(P<0.05).In the stratified analysis of blood pressure divided into non-hypertension,primary and secondary hypertension,tertiary hypertension,there were significant differences of serum endocan level in the overall blood pressure and diastolic pressure,with a nonlinear trend of falling first and then rising.Dividing the study population into the normal blood pressure and high-normal blood pressure group and hypertensive group,the logistic regression analysis showed that the odds ratio(OR)value of increasing endocan levels with each unit of systolic pressure increment was 1.043 [95% confidence interval(CI)=1.006~1.081] in the normal blood pressure and high-normal blood pressure group and the OR value of increasing endocan levels with each unit of systolic pressure increment was 1.033(95% CI=1.009~1.058)in the hypertensive group(all P<0.05).The OR value of increasing endocan levels with each unit of diastolic pressure increment were 0.70(95% CI=0.943~0.998,P<0.05)in the hypertensive group.In addition,the endocan level was not associated with the use of antihypertensive drugs.4.The endocan level was not associated with diabetes,carotid artery intima-media thickening,and lipid profiles(all P<0.05).Conclusions The level of serum endocan was 151.39(102.84,215.32)pg/ml in the community middle-aged and elderly population.Serum endocan level was negatively correlated with waist circumference in the total population,but were not related to sex,age,blood sugar,blood lipids and CIMT.In the total population,serum endocan level showed nonlinear trends with the change of blood pressure,presenting an upward trend from the normal blood pressure to high-normal blood pressure population.The serum endocan level then decreased from the high-normal blood pressure to primary hypertension population.The serum endocan level was on the rise again from the primary and secondary hypertension to tertiary hypertension population.
