Clinical Study Of Tonifying Kidney And Resolving Phlegm Combined With Acupoint Embedding In The Treatment Of Insulin Resistance In Adolescent Polycystic Ovary Syndrome

ObjectiveTo observe the effect of tonifying kidney and resolving phlegm combined with acupoint embedding in treating insulin resistance in adolescent Polycystic Ovary Syndrome.To evaluate its safety and to explore the function mechanism by the changes in menstrual cycle,body mass index(BMI),waist,waist-to-hip ratio(WHT),sex hormones(LH,T),glucose and lipid metabolism.To provide theoretical basis for further clinical application and popularization.MethodsIn this study,70 cases of insulin resistance in adolescent Polycystic Ovary Syndrome with kidney deficiency syndrome of phlegm and dampness that met the inclusion criteria were divided into 35 test and control groups by simple randomization.For three months,traditional Chinese medicine of tonifying kidney and resolving phlegm combined with acupoint embedding were given to the test group,while Metformin hydrochloride tablets were given to the control group during meals.All cases are directed to adjust lifestyle.Observe the changes in menstrual cycle,body mass index(BMI),waist,waist-to-hip ratio(WHT),sex hormones(LH,T),glucose and lipid metabolism(total cholesterol,triglycerides,fasting and 2-hour-post-meal blood-glucose,fasting and 2-hour-post-meal insulin,HOMA-IR).Then establish database,analyze statistic and evaluate the efficacy.ResultIn the treatment of obesity,both groups were able to reduce body mass index(BMI)and waist-to-hip ratio(WHT).The efficacy of the test group was better than that of the control group,and the difference was statistically significant(P<0.05).The effect of treatment on sex hormones showed total testosterone decreased in both groups,while the control group was superior to the test group.The difference was statistically significant(P<0.05).The level of LH decreased in both groups,but there was no statistically significant(P<0.05).The effect of treatment on glucose and lipid metabolism showed HOMA-IR decreased in both groups,and the test group was superior to the control group.The difference was statistically significant(P<0.05).There were significant differences of total cholesterol,fasting and 2-hour-post-meal blood-glucose and fasting and 2-hour-post-meal insulin when treatment finished in Intra-group comparison(P<0.05),but there were no significant differences in inter-group comparison(P>0.05).Comparing at different times,there were no significant differences of triglyceride and fasting blood-glucose(FBG)in the control group(P<0.05).However triglyceride and FBG were both decrease in the test group.And the differences were statistically significant(P<0.05)The total effective rate of improving menstrual disorders was 82.9% in the control group and 88.6% in the test group.There was no statistically significant difference between two groups(P>0.05).ConclusionIn reducing BMI and WHP,improving abdominal obesity and treating insulin resistance,the effect of tonifying kidney and resolving phlegm combined with acupoint embedding is better than that of Metformin hydrochloride.In reducing LH,promoting glucose and lipid metabolism and regulating the menstrual cycle,two treatments have similar efficacy.The effect of Metformin hydrochloride in reducing total testosterone is superior to that of tonifying kidney and resolving phlegm combined with acupoint embedding.But it is not clear whether total testosterone keeps going down after Metformin hydrochloride withdrawal,while in the test group shows it still reduces after stopping treatment.Therefor tonifying kidney and resolving phlegm combined with acupoint embedding has more lasting effect.Tonifying kidney and resolving phlegm combined with acupoint embedding in the treatment of insulin resistance in adolescent Polycystic Ovary Syndrome has remarkable and long-lasting effect,and also has no side effects.It is worth promoting and clinical application.
