ObjectiveThe clinical efficacy of catgut embedding at acupoints on patients with pre-diabetes(damp-heat accumulating spleen syndrome)was evaluated by observing the clinical effective rate,TCM syndrome integral,fasting blood glucose and other indicators.MethodsIn this study,66 patients who met the inclusion criteria were selected.The cases were from the physical examination center,acupuncture and moxibustion department,endocrinology outpatient and inpatient department of Hubei Provincial Hospital of Traditional Chinese Medicine.The random number table was used to divide them into acupoint embedding group and false embedding group,with 33 cases in each group.Both groups of patients were given basic life intervention,including health education,diet guide,and exercise.On this basis,the acupoint embedding group performed embedding operations at Weiwanxiashu,Pishu,Zusanli,Quchi,and Yinlingquan points,and implanted medical catgut into the needle tube once every 2 weeks.A total of 4 treatments,the course of treatment was 8 weeks.The false embedding group were the same as the embedding group,but there was no medical catgut implantation in the needle tube.The main observation indicators were clinical effective rate and TCM syndrome score.The secondary indicators included FPG,2hPG,HbA1 C,TC,TG,HDL-C,LDL-C.Compliance was evaluated using the "Pre-diabetes Self-Management Scale",and safety indicators included vital signs and adverse reactions.Results1.In this trial,1 case in the acupoint catgut embedding group withdrew due to pain intolerance,2 cases withdrew due to failure to treat on time,and 2 cases in the sham catgut embedding group withdrew spontaneously due to work reasons and unable to cooperate with treatment,1 case withdrew voluntarily due to traffic inconvenience,and finally the number of cases included in the analysis in the two groups was 30 cases.2.Comparison of general data before treatment: There was no statistical difference in the comparison of age,gender,glucose metabolism status,FPG,2hPG,HbA1 C,TC,TG,HDL-C,LDL-C,TCM syndrome scores of the two groups before treatment(P>0.05),with clinical comparability.3.Main outcome indicators:(1)Effective rate of clinical treatment:After treatment,the acupoint embedding group was effective in 10 cases,effective in 17 cases,and ineffective in 3 cases,with a total effective rate of 90.00%;in the false thread embedding group,4 cases were effective,13 cases were effective,and 13 cases were ineffective,the total effective rate was 66.67%;the difference in clinical effective rate between the two groups was statistically significant(P<0.05).(2)TCM syndrome score: After treatment,the TCM syndrome score of the two groups of patients decreased compared with before treatment,and the difference was statistically significant(P<0.05);the acupoint catgut embedding group improved the patient’s abdominal distension,body weight,drowsiness,and chest distress,Dry mouth and polydipsia were better than the false embedding group,and the difference was statistically significant(P<0.05),but after treatment,the two groups had no statistically significant differences in short yellow urine,poor appetite and poor appetite,and loose stools(P>0.05).4.Secondary outcome indicators:(1)Blood glucose related indicators:FPG and 2hPG levels of the two groups of patients decreased after treatment,and the difference was statistically significant(P<0.05),while Hb A1 C was not significantly improved(P>0.05);The comparison showed that the acupoint catgut embedding group was better than the false catgut embedding group in improving FPG and2 h PG,and the difference was statistically significant(P<0.05).(2)Blood lipid-related indicators: After treatment,the TC,TG,and LDL-C of the two groups were improved,and the difference was statistically significant(P<0.05),but the increase in HDL-C was not significant(P>0.05).The acupoint embedding group was better than the false embedding group in reducing TC,TG,LDL-C,and the difference was statistically significant(P<0.05).5.Compliance and safety evaluation: During the treatment,the vital signs of the two groups of patients were stable,and there were no adverse reactions such as hematoma,allergy,bleeding,fainting,hypoglycemia,and palpitations.In the acupoint catgut implantation group,only one patient withdrew from the experiment due to pain intolerance.ConclusionCompared with the false catgut embedding group,acupoint catgut embedding group can reduce FPG,2hPG,TC,TG,LDL-C levels in patients,improve blood glucose and blood lipid disorders,and relieve clinical symptoms of patients.Acupoint catgut embedding in the treatment of pre-diabetes(damp-heat accumulating spleen syndrome)has a good clinical effect,is safe and reliable,and is worthy of popularization and application. |
