Objective:The purpose of this study is to study 127 cases of ulcerative colitis(UC)by retrospective cohort study Objective to access the availability and security of traditional Chinese medicine in the treatment of active ulcerative colitis(UC),and to provide evidence-based medicine theoretical support for the real world research.Methods:In this study,retrospective cohort study method was used to collect patients with UC(mild to moderate)in the Department of spleen and stomach diseases from January 2018 to December 2019.According to the interval between two times of TCM treatment and the total cumulative time,the exposure level of patients was graded layer by layer,and they were divided into extremely high exposure,high exposure,low exposure and extremely low exposure.TCM combined with western medicine was used for conventional therapy The observation group(TCM exposure level is very high,high and low),The control group was treated with western medicine alone and the exposure level of traditional Chinese medicine was very low and all patients were followed up retrospectively.The total scores of TCM syndromes before and after treatment were recorded retrospectively,and the curative effect was evaluated before and after treatment;the long-term curative effect of the follow-up patients was evaluated: the ibd-q score of patients with inflammatory bowel disease,the total scores of TCM syndromes,the curative effect of main symptoms were evaluated,and the recurrence rate of the two groups and the relationship between different exposure levels and recurrence rate were analyzed.Results:(1)A total of 138 patients were included in this trial.During the follow-up,11 patients were lost due to the change of mobile phone number and unwilling to cooperate with the questionnaire survey.127 patients were actually followed up.(2)Baseline comparison: There was no significant difference in gender,type of disease,distribution of lesion scope,C-reactive protein,ESR,age,lesion time and disease grade before treatment between the two groups(P > 0.05).(3)Before and after treatment,the curative effect was evaluated1TCM syndrome total score evaluation: There is no difference between the two groups before treatment(P > 0.05),which can be compared;there is difference between the two groups after treatment(P < 0.05),the score of the observation group is lower than that of the control group,and the combination of Chinese and Western medicine has positive significance in improving TCM syndrome.2TCM syndrome efficacy evaluation: The markedly effective,effective and total effective rates of the observation group were 28.13%,60.93% and 89.06% respectively,while the markedly effective,effective and total effective rates of the control group were15.87%,58.73% and 74.6% respectively.There was difference between the two groups(P< 0.05).3Two groups of C-reactive protein efficacy evaluation: P < 0.05,the comparison between the two groups is different,indicating that the observation group can effectively reduce the level of C-reactive protein,but the control group is not as good as the observation group.4ESR efficacy evaluation of the two groups: There was no difference between the two groups(P > 0.05).It shows that there is no difference in the normal range of ESR between the two treatments.(4)Long term follow-up evaluation1Recurrence rate comparison: The recurrence rates of the observation group and the control group were 18.75% and 34.92% respectively,and there was a difference in the recurrence rate(P < 0.05),indicating that the use of traditional Chinese medicine in reducing the recurrence rate of such patients is significant.2IBD-Q score efficacy evaluation: There were significant differences in the total score of four dimensions between the two groups(P < 0.05).For the two symptom dimensions(intestinal and systemic)and two functional dimensions(emotional and social),the differences in each dimension were statistically significant(P < 0.05).It further shows that traditional Chinese medicine has certain positive significance in improving the quality of life of patients with this disease.3The main symptoms of curative effect evaluation: Two groups of patients in the comparison of the main symptoms,in which diarrhea,mucus purulent bloody stool,abdominal pain on the comparison of three symptoms are different(P < 0.05);in the comparison of symptoms of acute and severe,there is no difference(P > 0.05).4The total score of TCM syndrome: There was difference in the total score between the two groups(P < 0.05),and the score of the control group was significantly higher than that of the observation group,which indicated that the effect of using traditional Chinese medicine in improving the long-term curative effect of TCM syndrome was obvious.5The recurrence rate of different exposure levels in the observation group: 9.10% in the extremely high exposure group,11.76% in the high exposure group,and 36.80% in the low exposure group,the difference was statistically significant(P < 0.05),and the correlation coefficient was-0.275,indicating that the recurrence was negatively correlated with the exposure level,that is,the longer the time of taking Chinese medicine,the less the number of patients with recurrence,the lower the recurrence rate.(4)Safety evaluation: There was no obvious abnormality in the safety indexes of the two groups.Conclusion: The combination of traditional Chinese medicine and Western medicine in the treatment of active ulcerative colitis(mild to moderate)can significantly upgrade the clinical symptoms,Reduce the recurrence rate of the disease and Effectively enhance the quality of life of patients,with high safety and stable clinical efficacy.
