| BackgroundHelicobacter pylori(H.pylori)can lead to some serious diseases of the digestive system,such as peptic ulcer,gastric adenocarcinoma,etc.,and gastrointestinal diseases,such as idiopathic thrombocytopenic purpura,ischemic heart disease,metabolic syndrome,insulin resistance,etc.,are also closely related to H.pylori infection.About half of the world’s population is infected with H.pylori,especially in low-and middle-income countries with poor sanitation.With the gradual rise of antibiotic resistance,the eradication rate of H.pylori triple or quadruple regimen decreased gradually,and there were problems such as poor symptom relief and increased adverse reactions.Therefore,new drugs are needed to complement or replace antibiotics in the treatment of H.pylori infection.There is increasing evidence that probiotics can be used as an alternative or supplement to antibiotics in the treatment of gastrointestinal tract infections.After taking probiotics,H.pylori gastritis is improved and H.pylori colonization is reduced,which may reduce the use of antibiotics or reduce their side effects.ObjectiveTo evaluate the clinical application value of compound lactobacillus acidophilus combined with quadruple therapy in patients with chronic gastritis infected with H.pylori after initial treatment failure.Methods124 patients with chronic gastritis with H.pylori infection after initial treatment failure treated in the first affiliated Hospital of Zhengzhou University from October 2018 to June 2019 were randomly divided into observation group(n=62)and control group(n=62).The control group:amoxicillin+furazolidone+rabeprazole+bismuth potassium citrate tablets.The observation group was treated with compound lactobacillus acidophilus on the basis of the control group,and continued to take compound Lactobacillus acidophilus for 14 days after the course of treatment.The clinical efficacy,the eradication rate of H.pylori,the level of serum inflammatory factors and the incidence of adverse reactions were compared between the two groups.The patients were followed up for 1 year,and the recurrence rates of the two groups were compared.Results1.The total effective rate of clinical efficacy in the observation group and the control group was 93.55%(58/62)and 80.65%(50/62)respectively.The total effective rate in the observation group was significantly higher than that in the control group(χ2=4.593,P<0.05).2.The H.pylori eradication rates of the observation group and the control group were 88.71%(55/62)and 74.19%(46/62)respectively.The H.pylori eradication rate of the observation group was higher than that of the control group(x2=4.324,P<0.05).3.There was no significant difference in the levels of serum IL-1 β and IFN-γbetween the two groups before treatment(P>0.05),but the levels of serum IL-1 βand IFN-y in the two groups after treatment were lower than those before treatment(P<0.05),and the levels of IL-1 β and IFN-γ in the observation group were lower than those in the control group after treatment(P<0.05).4.The incidence of adverse reactions in the observation group and the control group was 4.84%(3/62)and 16.13%(10/62)respectively.The incidence of constipation,nausea,rash and other adverse reactions in observation group was lower than that in control group(χ2=4.211,P<0.05).5.During the one-year follow-up,the recurrence rate of H.pylori in observation group and control group was 7.27%(4/55)and 28.26%(13/46),respectively.The recurrence rate of observation group was lower than that of control group(χ2=7.882,P<0.05).ConclusionsCompound lactobacillus acidophilus combined with quadruple therapy in the treatment of H.pylori infection chronic gastritis,which failed in initial treatment,can significantly improve the clinical symptoms,improve the eradication rate of H.pylori,have good safety and can reduce the recurrence rate,and have certain clinical application value. |
