Objective:1.To quantitatively evaluate the relationship between liver fat content and the grade of acute pancreatitis by QCT,and the relationship between the changes of liver fat content before and after treatment and prognosis2.To explore the application value of QCT in the evaluation of fatty liver.Materials and Methods:The patients with acute pancreatitis who were hospitalized in the First Affiliated Hospital of Zhengzhou University from December 2019 to December 2020 were prospectively collected.The original CT images were obtained by Siemens SOMATOM force dual source CT machine and Mindways QCT phantom.At the same time,healthy adults who were hospitalized in our hospital for physical examination or bone mineral density examination and underwent upper abdomen or whole abdomen plain scan on the same CT machine under the same scanning conditions were selected as the control group.In this study,108 subjects were included.The general information and laboratory indexes of all subjects were collected,and whether the acute pancreatitis group was improved before and after treatment was observed.The liver fat content was measured by QCT Pro post-processing workstation,and the liver CT value and spleen CT value were measured on the original image.The difference of liver and spleen CT value and the ratio of liver and spleen CT value were calculated.The relationship between liver fat content and acute pancreatitis grade,as well as the changes of liver fat content,liver CT value and triglyceride level before and after treatment were analyzed,The relationship between liver fat content and CT value of liver,the difference of CT value of liver and spleen,the ratio of CT value of liver and spleen,and the severity of fatty liver were evaluated.Results:1.The study finally included 108 subjects,54 cases in the acute pancreatitis group,and 54 cases in the control group.Among them,26 cases in the acute pancreatitis group completed the CT reexamination within 20 days.2.The CT value of liver and the ratio of CT value of liver and spleen in the acute pancreatitis group was lower than that in the control group,and the liver fat content measured by QCT was higher than that in the control group,the difference was statistically significant(P<0.05).Balthazar CT score was positively correlated with liver fat content.Balthazar CT Score and liver fat content were correlated with serum triglyceride level.After treatment,the CT value of liver,the ratio of CT value of liver and spleen,the content of liver fat and the level of serum triglyceride were decreased compared with those before treatment,the difference was statistically significant(P<0.05).After treatment,the patients with acute pancreatitis were improved in varying degrees(Clinical symptoms were relieved,CT grading was decreased).3.In the acute pancreatitis group and the control group,the liver fat content measured by QCT had a good correlation with the liver CT value,the difference between liver and spleen CT value and the ratio of liver and spleen CT value,and was positively correlated with the severity of fatty liver.The intra group correlation coefficients of liver fat content measured by QCT were all greater than 0.75.Conclusions:1.The liver fat content measured by QCT is positively correlated with the grade of acute pancreatitis.The liver fat content measured by QCT can be used to evaluate the severity and prognosis of acute pancreatitis.2.The combination of QCT and conventional CT can realize the early,real-time,quantitative and reliable evaluation of fatty liver.
