| Objective:Through the clinical observation and research on the treatment of anal sinusitis due to damp-heat downward flow with modified Zhitong Rushen Decoction combined with compound Ningzuo suppository(in-hospital preparation),the clinical effect of this method on anal sinusitis due to damp-heat downward flow was discussed,which provided a new idea and method for clinical treatment of anal sinusitis due to damp-heat downward flow.Methods:A total of 72 patients with dampment-heat injection anal sinusitis who were admitted to the Third Affiliated Hospital of Yunnan University of Traditional Chinese Medicine(Kunming Hospital of Traditional Chinese Medicine)from December 2019 to February 2021 were selected.The patients were randomly divided into experimental group(n = 36)and control group(n = 36).The control group was treated with compound Ningzole suppository(hospital preparation)in the morning and evening,and the experimental group was treated with Jiawei Zhitong Rushen decoction(once a bag,3 times a day)on the basis of compound Ningzole embolization of the anus,and the treatment time was 2 weeks as a course of treatment.By inquiring about the patients’ symptoms,anal finger examination,microscopic examination and three-dimensional anorectal ultrasound(intracavitary ultrasound)were performed,and the quantitative integral method was used to evaluate the changes of clinical symptoms and signs syndromes and imaging results before and after treatment.and the changes of patients’ condition were recorded one month after drug withdrawal.The changes of the above indexes before and after treatment were observed,and the data were statistically analyzed.Results:(1)Baseline analysis: 7 cases were dropped and eliminated during the treatment process and after the return visit,and the final remaining effective cases were 65 cases,including 33 cases in the experimental group and 32 cases in the control group.Before the treatment,the age,sex,course of disease,symptoms,signs and imaging conditions of the two groups were compared.After statistical treatment,P >0.05 was comparable.(2)Comparison of the total efficacy of symptoms and signs: after treatment,the total effective rate of the experimental group was 87.88%,and that of the control group was68.75%.By rank sum test,the efficacy of the experimental group was better(P<0.05).Among them,the scores of clinical symptoms(anal pain,anal swelling,anal pruritus,lack of defecation)and signs(anal sinus tenderness,anal recess congestion and edema)were compared between the two groups,and the differences were statistically significant(P<0.05),indicating that the experimental group was better than the control group in improving clinical symptoms and signs.(3)Comparison of imaging effect: the two groups were examined by intracavitary ultrasound after treatment.Data statistics showed that the total effective rate of imaging effect was 81.82% in the test group and 56.25% in the control group.Statistics showed that P < 0.05,indicating that the intraluminal ultrasound changes were better in the experimental group.(4)Comparison of recurrence and adverse reactions: one month after drug withdrawal,the recurrence of the two groups was observed.Statistical test showed that the control group was more likely to relapse(P < 0.05).No adverse events occurred in both groups during the treatment.Conclusions:(1)Modified Zhitong Rushen Decoction combined with Compound Ningzuo Suppository is effective in treating anal sinusitis of downward damp-heat type,which can effectively relieve anal symptoms,improve anal signs and reduce the area and number of low-density echo areas under intracavitary ultrasound.;(2)Modified Zhitong Rushen Decoction combined with Compound Ningzuo Suppository has low recurrence rate and high safety in treating anal sinusitis with damp-heat downward flow,which is worthy of clinical promotion. |
