Clinical Study On Zhigancao Decoction For Treatment Of Chronic Heart Failure Combined With Atrial Fibrillation With Deficiency Of Yin And Yang

Research purposes:By observing and comparing the changes of heart function index,symptom rating of atrial fibrillation,TCM symptom and quality of life index of patients with heart failure complicated with atrial fibrillation,this topic explores the effective way of treating chronic heart failure complicated with atrial fibrillation by combining traditional Chinese and western medicine.Research methods:This project patients come from the Kunming Traditional Chinese Medicine Hospital,from November 2019 to October 2020.The treatment group was additionally treated with zhigancao decoction orally on the basis of the standardized treatment of conventional heart failure combined with atrial fibrillation.The control group received standardized treatment of conventional heart failure combined with atrial fibrillation.The treatment course of the two groups was unified for 12 weeks.The changes of physical and chemical indexes of cardiac function,symptom rating of atrial fibrillation,scores of TCM symptoms and scores of all dimensions in the quality of life assessment table were observed and recorded before and after treatment in two groups of patients.The collected research data were statistically analyzed and processed.Research Results:1.Physical and chemical indexes of heart function in 2 groups: BNP,mean ventricular rate and resting ventricular rate in 2 groups were significantly improved before and after treatment(P < 0.01),while there was no difference between 2 groups after treatment(P >0.05).LAD,LVDD and LVEF were significantly improved in both groups before and after treatment(P < 0.05),but there was no difference between the two groups after treatment(P > 0.05).2.Lee’s heart failure score was significantly improved in both groups before and after treatment(P < 0.05),but there was no difference between the two groups after treatment((P > 0.05).The 6-minute walking distance and intra-group comparison before and after graded treatment were significantly improved in 2 groups(P < 0.01),but the comparison between groups after treatment was better than that in the control group(P < 0.05).NYHA cardiac function grade and atrial fibrillation symptom grade in 2 groups were significantly improved before and after treatment within the group(P < 0.05),but after treatment,the effective rate in the treatment group was significantly higher than that in the control group(P < 0.01).3.In terms of scores of TCM symptoms of patients in both groups,palpitation,shortness of breath,fatigue,sweating,dry mouth and chills were significantly improved in the treatment group before and after treatment(P < 0.01),dizziness and somnolence were significantly improved in the treatment group(P < 0.05).In the control group,palpitation,shortness of breath and fatigue were significantly improved before and after treatment(P <0.01),and in the control group,dry mouth,dizziness,dark complexion,chills and sweating were significantly improved before and after treatment(P < 0.05).Comparison between the two groups: after the treatment of palpitation,shortness of breath,fatigue and sweating,the improvement in the treatment group was significantly better than that in the control group(P < 0.01);after the treatment of dry mouth and chills,the improvement in the treatment group was significantly better than that in the control group(P < 0.05);after the treatment of dizziness and dullness,there was no difference between the two groups(P > 0.05).Both groups had improved TCM symptoms,but the treatment group had better effect on palpitation,shortness of breath,fatigue,sweating,dry mouth and chills.4.Quality of life: The scores of all dimensions in the quality of life table of 2 groups were significantly improved within the group before and after treatment,the difference was very significant(P < 0.01).After the treatment of somatic dimension,the improvement of the treatment group was significantly better than that of the control group(P < 0.05),but there was no difference in other dimensions(P > 0.05).5.Safety: Zhigancao decoction had no effect on the liver and kidney functions of the subjects,and there was no difference between the groups after the test(P > 0.05).Research Conclusion:1.Zhigancao decoction can further improve the clinical symptoms of patients with chronic heart failure complicated with atrial fibrillation,and can improve the patients’ 6-minute walking distance,New York cardiac function grade,and quality of life.2.Zhigancao decoction can improve the somatic quality of life of patients with heart failure complicated with atrial fibrillation.3.Zhigancao decoction is safe for patients with heart failure complicated with atrial fibrillation.
