| Background:Ovarian cancer is the most fatality rate tumor among gynecological malignancy,and it is also the main cause of tumor-related death in women.The main cause of treatment failure or even death in patients with ovarian cancer is the generation of platinum drug resistance,but there is no effective treatment for patients with platinum drug resistance.Objective:This study identified clinical factors related to platinum resistance of ovarian cancer by retrospective analysis.Close follow-up to observe patients with high possibility of platinum resistance recurrence,and find the monitoring indicators and methods that can effectively prevent platinum resistance recurrence of ovarian cancer.Methods:From January 2013 to December 2020,105 patients with ovarian cancer who met the requirements of this study were selected for standardized treatment in the first Hospital of Jilin University,which were divided into platinum-sensitive recurrent ovarian cancer group and platinum-resistant recurrent ovarian cancer group.Compare the differences between the two groups in age,ascites,pathological type,FIGO stage,neoadjuvant chemotherapy,cancerous embolus in the blood vessel or lymphatic vessel,CA125 level at the time of initial diagnosis,serum albumin levels,AGR、NLR、PLR、LMR,and further analyze the factors that affect the recurrence of platinum resistance in ovarian cancer.Results:1.This study included 105 patients with ovarian cancer,83 patients(79%)were diagnosed as advanced stage(Ⅲ、Ⅳ)in tne initial clinical diagnosis.2.Single factor analysis showed that there were significant differences between platinum-resistant recurrent ovarian cancer group and platinumsensitive recurrent ovarian cancer group in the initial diagnosis of ascites volume,ascites with or without cancer cell,FIGO stage,cancerous embolus in the blood vessel or lymphatic vessel,serum albumin levels,AGR、PLR、LMR.3.Single factor analysis showed that there were no significant differences in age,pathological type,neoadjuvant chemotherapy,CA125 level and NLR between platinum-resistant recurrent ovarian cancer group and platinum-sensitive recurrent ovarian cancer group.4.Multivariate logistic regression analysis showed that FIGO stage(P=0.023、OR=7.848、95%CI:1.321~46.621),cancerous embolus in the blood vessel or lymphatic vessel(P=0.041、OR=0.323、95%CI:0.109~0.957),LMR(P=0.019、OR=0.205、95%CI:0.054~0.768)were the independent risk factors associated with platinum resistance recurrent ovarian cancer.Conclusions:1.Ascites volume,ascites with or without cancer cells,serum albumin levels,AGR,PLR are associated with platinum resistance recurrence,but are not independent risk factor of platinum resistance recurrence in ovarian cancer.2.FIGO stage(Ⅲ、Ⅳ),cancerous embolus in the blood vessel or lymphatic vessel(positive),LMR(<5.12)are associated with platinum resistance recurrence and are independent risk factors of platinum resistance recurrence in ovarian cancer.3.There are no significant difference in age,pathological type,neoadjuvant chemotherapy,CA125 level at initial diagnosis and NLR in this study.And we need to expand the sample size for further study. |
