Objective:Compile the Discharge Readiness assessment Scale for Adults with Type 2Diabetes,and test its reliability and validity to provide a scientific and effective assessment tool for medical staff to evaluate patients’ discharge readiness,and promote objective and scientific improvement of patient discharge plans Process,realize the coordination between patient discharge preparation and personalized nursing,ensure that patients realize effective self-management after hospital,improve the level of blood glucose management after hospital,and provide guidance for clinical nursing health education.Methods:The literature review method,group discussion method,Delphi method and questionnaire survey methods were used to compile the Type 2 Diabetes Discharge Readiness Assessment Scale.By searching the domestic and foreign databases,guide websites and related diabetes professional association websites from January 2010 to January 2020,and referring to the 2017 International Diabetes Federation’s global guideline evaluation,5 guides with a score of 60 or more were included;related randomized controlled trial literature 116 In this article,there are 10 assessment scales for related patients’ discharge readiness and self-management level,which are guided by the theory of " knowledge-attitude-practice " to form an initial scale.The Delphi method was used to conduct two rounds of expert letter inquiries to form an evaluation scale.The scale covers four dimensions of knowledge,belief,behavior,and physical readiness,including five sub-dimensions of diet therapy,exercise therapy,drug therapy,self-monitoring and psychosocial support,and contains a total of 32 items.Selected from August to October 2020,230 pre-discharged patients with diabetes in the Department of Endocrinology and Metabolism of a tertiary hospital in Changchun City,distributed a questionnaire containing general information and an evaluation scale.The survey results show that this scale has good reliability and effectiveness.Results:1.This research is guided by the theory of " knowledge-attitude-practice ",and finally formed the "Adult Type 2 Diabetes Patient Discharge Readiness Assessment Scale",covering four dimensions of knowledge,belief,behavior,and physical readiness,The five sub-dimensions of diet therapy,exercise therapy,medication treatment,self-examination and psychosocial support,contain 32 items in total.2.The reliability and validity test results of the assessment scale show that the correlation coefficient between the scores of each item and the total score ranges from0.557 to 0.758,and the CR value of all items in this scale ranges from 7.792 to 12.036,and were statistically significant(P<0.05),according to the results of the item analysis delete item A3,and keep the remaining items for factor analysis.The Bartlett sphere analysis was performed on the scale,the KMO value was 0.919,the Bartlett approximate X~2 value was 5683.675,and P<0.001 can be used for factor analysis.The first exploratory factor analysis delete item A1,and no items will be deleted after exploratory factor analysis again.After the second exploratory factor analysis,4common factors were extracted,the contribution rates were 21.916%,20.429%,16.214%,7.343%,the cumulative contribution rate was 66.993%,and the factor loading of each item was 0.584~0.833.The content validity of the scale is: S-CVI range is 0.79~1,S-CVI/UA is 0.81,and S-CVI/Ave is 0.95.The Cronbach’s αcoefficient of the total scale is 0.955,and the half-reliability is 0.846.Reliability and validity test results show that this scale has good reliability and validity.Conclusion:The "Type 2 Diabetes Patient Discharge Readiness Assessment Scale" compiled by this research is scientific and effective.It has been verified to have good reliability and validity,which will help medical staff to evaluate patient’s actual status of hospital discharge readiness scientifically and systematically,which can provide guidances to improve the effect of clinical nursing health education. |
