Objective: To investigate the effect of Goal-Setting Theory on knowledge, attitude and behavior intervention in patients with Type 2 Diabetes, to investigate the Goal Setting intervention to promote the applicability of the knowledge, attitude and behavior change in patients with Type 2 Diabetes in domestic, to investigate the effective health education management model to adapt to the high incidence of diabetes in our country under the present situation.Methods: The research subjects were 200 cases of hospitalized patients with Type 2 Diabetes selected in a Changchun top three hospital endocrinology from October 2014 to March 2015. In the case of patients’ informed consent, we fill in forms such as the diabetic patients with general information questionnaire and Audit of diabetes’ knowledge and Diabetes Attitude Scale and Diabetes self-care Activities. After admission, the patient information was collected and analyzed, and the individual education guidance was supplied according to the theory of Goal Setting, after discharge, we would follow-up every 4 weeks. After 12 weeks of follow-up, we would fill out the questionnaire data again and review relevant indicators at the same time. The faith in patients with diabetes knowledge, attitude and behavior before and after effect through goal setting were analyzed. Data analysis, the quantitative data with mean ± standard deviation or the median ± inter-quartile range description, goal setting comparison before and after intervention using t test or rank and inspection; Adoption or forming than describing count data, comparison before and after intervention using chi-square test, the method of data analysis of statistical description.Results: 1. A total of 200 patients finished the research, among which 147 were male, female 53. 2. During the study period, a total of 258 items to set, including diet goals in 93 cases, 106 cases of moving object, 24 cases of monitor target, 22 cases of foot care, 6 cases of drug application goal, 7 cases of other goals, mainly in hypoglycemia prevention and drinking. Goal setting way of independent targets set for 85 cases, cooperation for 137 cases, educators specify the target for 36 cases. 3. The patients’ diabetes knowledge level were higher in all dimensions after goal setting, and the difference was statistically significant(P < 0.001), Patients with diabetes after goal setting attitude in each dimension, the necessity of "professional training", "Type 2 Diabetes Mellitus severity", "strict control of blood sugar value" and "the psychological impact of diabetes" score were higher than the target set before, and the difference was statistically significant(P < 0.001),goal setting in patients with diabetes after the protection act, "diet", "sports", "monitoring" and "foot care" score were higher than before the goal setting, and the difference was statistically significant(P < 0.001). 4. In the process of follow-up and feedback, in addition to the normal follow-up, 74 of patients take the initiative to consult, 126 of patients mainly through passive guidance; 64.00 % of the patients believed goal setting is helpful to promote patients behavior change, 36.00 % of patients thought a little help.64.50% of patients thought it was not difficult to use goal setting, 76.00 % of patients were willing to continue to use this method and recommend this method to others.Conclusion: 1. Goal setting can promote the patients to understand the disease related knowledge in active or passive way, with the accumulation of knowledge, eventually the overall cognitive level of the patients with diabetes is improved from the breadth and depth. Goal setting can make the understanding of the diabetes patients attitude change. Goal setting can make the patients’ self-care behavior change on the premise of effective control of the disease. 2. Goal setting has applicability to the diabetes education and management. 3. Goal setting can arouse the enthusiasm of patients, and patients agree with it and are willing to use this method.
