ObjectiveThis study was conducted to investigate the effectiveness of time-based acupressure on the clinical symptoms of elderly patients with hyperactive liver yang in vertigo(essential hypertension)under the guidance of the theory of Zi Wu Liu Chuan,in order to provide a time-based care service with Chinese medicine characteristics,with a view to providing a reference for clinical care practice and later clinical care studies of elderly patients with hyperactive liver yang in vertigo(essential hypertension).MethodsIn this study,elderly patients with vertigo of the suprahepatic Yang type who were hospitalized in the geriatric department of a tertiary care Chinese medicine hospital in Kunming from December 2019 to September 2020 and who had a Western medical diagnosis of grade I or grade II hypertension were selected;a total of 76 cases were included and randomly divided into two groups,the experimental group and the control group,with 38 patients in each group.In the control group,regular clinical care was given along with medication to lower blood pressure: blood pressure monitoring,medication guidance,emotional and psychological care,dietary care and daily living care.In the experimental group,based on the theory of Zi Wu Liu Yin,acupuncture points on the Foot Tai Yin Spleen Meridian and the Foot Shao Yin Kidney Meridian were selected and massaged by pressing and kneading at the time when these meridians were in season.In the morning,when the Spleen meridian is in season(9-11am),the Sanyinjiao and Tai Bai points on the Spleen meridian and the Fenglong point on the Stomach meridian are chosen as the main points,together with the Tai Chong and Xingma points on the Liver meridian and the Baihui and Sun points on the head,and then in the afternoon,when the Kidney meridian is in season(17-19pm),the Yongquan,Taixi and Fuyao points on the Kidney meridian are chosen as the main points,together with the Tai Chong and Xingma points on the Liver meridian and the Baihui and Sun points on the head.The acupressure points on the Liver meridian are combined with the Tai Chong and Xingma points on the Liver meridian,and the Baihui and Sun points on the head.The massage was performed once a day for 20 minutes at the peak of qi and blood flow in the Foot Taiyin Spleen and Foot Shao Yin Kidney meridians for 2 weeks.The TCM symptom score,vertigo disorder degree score,blood pressure value and clinical efficacy of the two groups were observed and recorded before and after the intervention,and the results of the two groups were statistically analysed using SPSS 25.0,so as to compare and evaluate the intervention effects of the two intervention methods.Results1 Comparison of the TCM symptom scores of vertigo:(1)Comparison within the group:the differences in the single symptom scores and total scores of vertigo,headache,dry mouth and bitter mouth between the test group before and after intervention were statistically significant(P<0.05);the differences in the other TCM symptom scores were not statistically significant(P>0.05).In the control group,the differences were statistically significant(P<0.05)when comparing the single symptom points and total points of vertigo and headache before and after the intervention.(2)Comparison between groups: the differences in the single symptom scores and total scores of vertigo,headache,irritability,dry mouth and bitter mouth between the two groups after intervention were statistically significant(P<0.05);the differences in the remaining TCM symptom scores of vertigo were not statistically significant(P>0.05).2 Comparison of the degree of dizziness impairment(DHI)scores:(1)Intra-group comparison: There was a statistically significant difference between the degree of dizziness impairment scores before and after the intervention in the control group(P<0.05);there was a statistically significant difference between the degree of dizziness impairment scores before and after the intervention in the test group(P<0.05).(2)Comparison between groups: The difference was statistically significant(P<0.05)when comparing the scores of the degree of vertigo disorder between the two groups after the intervention.3 Comparison of blood pressure values:(1)The difference between the systolic blood pressure of the control group and the test group on day 7 of the intervention was statistically significant(P<0.05);the difference between the systolic blood pressure of the control group and the test group on day 14 of the intervention was statistically significant(P<0.05).(2)The difference was statistically significant(P<0.05)when comparing the diastolic blood pressure of the control group and the test group on day 14 of the intervention.4 Comparison of clinical efficacy: After 14 days of intervention,the total effective rate of clinical efficacy was 63.2% in the control group and 84.2% in the test group,with statistically significant differences(P<0.05),indicating that the clinical efficacy of the test group was better than that of the control group.Conclusion1 Timing-based acupressure based on the Ziwu Liujiao theory has improved the clinical symptoms of elderly patients with vertigo(essential hypertension)in the form of hyperactivity of liver and Yang,with more significant effects on vertigo,headache,irritability,dry and bitter mouth.2 Timing-based acupressure based on the Zi Wu Liu Chuan theory has an adjunctive effect on the reduction of Class I and Class II blood pressure values in elderly patients with vertigo(essential hypertension)with suprahepatic Yang.3 Time-based acupressure based on the Ziwu Liujing theory can reduce the degree of dizziness and improve the quality of life of elderly patients with vertigo(essential hypertension)with suprahepatic Yang.
