Effect Comparation Of Ovulation Induced By Acupuncture In PCOS And EMT

Objective:One of the main causes of growing incidence of infertility is ovulation obstacle which can caused by polycystic ovary syndrome(PCOS)and endometriosis(EMT).For the better use of acupuncture,this research compared the clinical effect of the acupuncture method of inducing ovulation in ovulation obstructive infertility caused by PCOS and EMT to provide safer and more effective treatment for the ovulation obstructive infertility.Methods:After preprocessing,when the diameter of follicles was more than 18*18 mm monitored by B-mode ultrasonography,120 cases who met the inclusion criteria were divided into HCG control group and acupuncture experimental group randomly,according to basic diseases experimental group was divided into two subgroups:PCOS group and EMT group.After control group was given 5000 iu of HCG intramuscular,experimental group was given acupuncture to induce ovulation,the research compared the the level of estradiol and progesterone in the middle of the luteal phase,ovulation rate.luteinization(LUFS)rate,ovarian hyperstimulation(OHSS)rate and clinical pregnancy rate between two main groups and two subgroups.Results:(1)Baseline conditions:there was no statistically significant difference between each group in age,course of infertility and primary disease categories(P>0.05).(2)Ovulation conditions:69 menstrual cycles were observed in the PCOS group,including 47 ovulation cycles,with an ovulation rate of 68.12%;75 menstrual cycles were observed in the EMT group,including 38 ovulation cycles,with an ovulation rate of 50.67%;144 menstrual cycles were observed in the acupuncture group,including 85 ovulation cycles,with an ovulation rate of 59.03%;136 menstrual cycles were observed in the HCG group,including 80 ovulation cycles,with an ovulation rate of 58.82%.The ovulation rate of the PCOS group in acupuncture group was higher than the EMT group(P<0.05).(3)Pregnancy conditions:28 cases were observed in the PCOS group,15 cases were pregnant,the clinical pregnancy rate was 53.57%;30 cases were observed in the EMT group of.8 cases were pregnant,the clinical pregnancy rate was 26.67%;58 cases were observed in the acupuncture group,23 cases were pregnant,the clinical pregnancy rate was 39.66%;59 cases were observed in the HCG group.22 cases were pregnant,the clinical pregnancy rate was 37.29%;The clinical pregnancy rate of PCOS group in acupuncture group was higher than the EMT group(P<0.05).(4)The level of estradiol(E2)in the middle of the luteal phase:the PCOS group E2:02.52±72.17 pg/mL.the EMT group E2:174.80±71.35 pg/mL;the acupuncture group E2:188.08±72.83 pg/mL.the HCG group E2:198.35±60.16 pg/mL.The E2 level of the PCOS group in acupuncture group was higher than the EMT group(P<0.05).(5)The level of progesterone(P)in the middle of the luteal phase:the PCOS group P:14.78±8.87 ng/mL,the EMT group P:11.38±7.42 ng/mL;the acupuncture group P:13.12±8.34 ng/mL,the HCG group P:12.78±8.11 ng/mL.The P level of the PCOS group in acupuncture group was higher than the EMT group(P<0.05).(6)The occurrence of LUFS:69 cycles were observed in the PCOS group,including 5 cycles of LUFS,with the rate of 7.25%,75 cycles were observed in the EMT group,including 14 cycles of LUFS,with the rate of 18.67%,144 cycles were observed in the acupuncture group,including 19 cycles of LUFS,with the rate of 13.19%,136 cycles were observed in the HCG group,including 35 cycles of LUFS,with LUFS rate of 25.74%.The LUFS rate of the acupuncture group was lower than the HCG group(P<0.05),meanwhile the LUFS rate of the PCOS group in acupuncture group was also lower than the EMT group(P<0.05).(7)The occurrence of OHSS:There was no OHSS happened in acupuncture group,136 cycles were observed in the HCG group,including 6 cycles of OHSS,with the rate of 4.4%.the OHSS rate of the acupuncture group was lower than the HCG group(P<0.05).Conclusion:(1)Observing the ovulation obstructive infertility caused by two kinds of diseases(PCOS&EMT),acupuncture inducing ovulation treatment of PCOS ovulation obstructive infertility has better curative effect,which canincrease the ovulation rate and clinical pregnancy rate,improve the the level of estradiol and progesterone in the middle of the luteal phase,lower the incidence of LUFS and OHSS.(2)The acupuncture method of inducing ovulation in ovulation obstructive infertility caused by PCOS and EMT has both achieved satisfactory curative effect.When compared to the use of HCG,acupuncture has safe and effective characteristics and advantages,which is worthy of clinical application.
