The Tutor Inherits The Experience Of Treating Palpitations With Traditional Chinese Medicine Of YE Tianshi

Objectives:In this study,Auxiliary Platform of Traditional Chinese Medicine Inheritance is used to conduct statistics on the medical cases of palpitations treated by Ye Tianshi and my tutor Chen Jingwei.In this way,the prescription ideas,treatment principles,treatment methods and drug characteristics can be obtained,so as to study the inheritance and development of the experience of treating palpitations by tutor,refine the characteristics of Wumen traditional Chinese medicine in the treatment of palpitations,and guide clinical medication.Methods:Collect palpitation cases included in Guide to Clinical Practice with Medical Records etc by Ye Tianshi’s medical case collection and outpatient medical cases of palpitation patients treated by my tutor Chen Jingwei in the department of cardiology of Suzhou hospital of Chinese medicine from 2018 to 2020.The medical cases are recorded into the database based on TCM inheritance auxiliary platform,and the data mining methods such as frequency analysis,correlation analysis and entropy level clustering analysis are used to analyze the experience of the two patients in the treatment of palpitation.Results:By collating 91 medical cases of palpitations and 92 prescriptions of Ye Tianshi,the top two concomitant symptoms other than palpitations were sleeplessness and vertigo.Ye Tianshi liked to use Fushen,Renshen,Zhi Gancao,Baishao and Sheng Dihuang to treat palpitations,and the four qi of the drugs were mostly warm,flat,and cold;the five tastes of the drugs were mostly sweet,bitter,and pungent;and the attributive meridians mostly belonged to the kidney,heart,and liver meridians.Based on the association rule analysis,four pairs of commonly used drug pairs were extracted:Renshen-Fushen,Zhi Gancao-Baishao,Sheng Dihuang-Ejiao,Dazao-Guizhi-Zhi Gancao.Based on entropy hierarchical cluster analysis,eight core drugs such as Zhi Gancao,Baishao and Dazao were extracted as well as four new candidate prescriptions such as Zhi Gancao,Baishao,Dazao,Sheng Dihuang,Ejiao,Maidong were extracted.By collating 121 cases of palpitations by Chen Jingwei,my tutor liked to treat palpitations with Wu Weizi,Longgu,Tai Zishen,Suan Zaoren,Zhi Gancao,and Maidong,etc.The four gases of the drugs were mostly warm,flat,and cold,and the five tastes of the drugs were mostly sweet,bitter,and pungent,and the attributive meridians mostly belonged to the heart,spleen,and liver meridians.Based on the association rule analysis,it was found that the commonly used drug pairs were dominated by princely Tai Zishen-Wu Weizi,Longgu-Wu Weizi,Maidong-Wu Weizi,Maidong-Tai Zishen,Suan Zaoren-Wu Weizi,and Longgu-Muli.Based on entropy hierarchical cluster analysis,10 core drugs such as Fushen,Danggui,Yuanzhi as well as 5 new candidate prescriptions including Fushen,Danggui,Yuanzhi,Zhike,Suan Zaoren were extracted.Conclusion:By examining the prescriptions and medication ideas of Ye Tianshi and my tutor for palpitations,it was concluded that they share the same lineage of medication,that is,they advocate "weakness of heart qi" as the basis for the onset of palpitations,focus on "tonifying deficiency and consolidating the root" and "calming the mind and nourishing the heart" as the rule of treatment,take tonic as the main treatment principle for palpitation and focus on"lightness,clarity,flexibility,and ingenuity" in the use of medicine.On this basis,tutor developed the treatment of palpitation.However,they differed slightly in their treatment methods:Ye Tianshi focused on tonifying the deficiency and consolidating the root,making good use of qi-yin double tonics while tutor pays more attention to the treatment methods of"tonic,nourishment,restraint and move",which means he flexibly uses the methods of"relieving uneasiness of mind,harmonizing qi,promoting blood circulation and removing blood stasis".On the thought of prescriptions,both have their own focus.Ye Tianshi advocates"treating heart and kidney together,communicating heart and kidney" while my tutor advocates"treating heart and spleen together,treating heart and liver together".
