Objective: To analyze the influencing factors of 1-year survival after radical pancreatectomy,and to evaluate the predictive value of neutrophil lymphocyte ratio(NLR)and platelet lymphocyte ratio(PLR)for 1-year survival after radical pancreatectomy.Methods: The basic data and follow-up results of 127 patients who received radical pancreatic carcinoma resection and were pathologically cleared with pancreatic ductal adenocarcinoma(PDAC)in the general surgery department of Wannan Medical College,Yi Jishan Hospital from January 2012 to December 2019 were retrospectively analyzed.Calculate the NLR and PLR values through the patient’s blood routine,use the ROC curve to select the boundary value with the optimal index,and divide the patient into low NLR group(≤2.87)and high NLR group(>2.87),low PLR group(≤147)and high PLR group(>147).Through univariate and multivariate analysis of the factors that affect the 1-year survival after radical pancreatic cancer resection,the predictive value of NLR and PLR in the 1-year survival of patients with pancreatic cancer undergoing radical pancreatic cancer resection was evaluated.Results: Among the 127 patients,57(44.9%)had a survival time of more than 1 year after surgery.The results of univariate analysis showed that age,gender,surgical methods,tumor location,and size had no effect on the 1-year survival period of patients with pancreatic cancer after radical pancreatectomy.Tumor differentiation,lymph node metastasis,postoperative chemotherapy,TNM staging,NLR and PLR have influence on1-year survival of patients with pancreatic cancer after radical pancreatectomy.COX regression multivariate analysis was performed on the influencing factors in the univariate analysis.The results showed that the 1-year survival rate of high NLR group was 32.6%,which was lower than 61.4% of low NLR group,and the difference was statistically significant(P<0.001).The 1-year survival rate of high PLR group was 28%,which was lower than that of low PLR group(55.8%),but the difference was not statistically significant(P >0.05).Postoperative chemotherapy has a certain effect on the 1-year survival rate of pancreatic cancer patients underwent radical resection(HR=1.650,95%CI 0.999-2.728,P=0.051).Conclusion: 1.The preoperative NLR=2.87 can be used as the truncation value to affect the 1-year survival after radical resection of pancreatic cancer patients.Patients with preoperative NLR < 2.87 have a higher 1-year survival than those with NLR >2.87.2.The preoperative PLR=147 can be used as a cutoff value to affect the 1-year survival after radical resection of pancreatic cancer.The 1-year survival rate of patients with preoperative PLR≤147 was higher than that of patients with PLR > 147.3.Preoperative NLR and PLR have an impact on the 1-year survival period of patients with pancreatic cancer after radical pancreatic cancer resection,and NLR has a certain predictive value for the prognosis of patients.
