Study On The Correlation Between TCM Syndrome Type And VIP And SP Levels In Acute Stage Of Ischemic Stroke

Objective:The studies by detecting the expression levels of brain intestinal peptide vasoactive intestinal polypeptide(VIP)and substance p(SP)in serum of patients with different TCM syndrome types in the acute stage of ischemic stroke,to analyze the correlation between the expression levels of VIP and SP and the TCM syndrome types in the acute stage of ischemic stroke,the aim is to seek the objective basis for the syndrome differentiation of acute stage of ischemic stroke.Methods:Clinical collection of patients in acute stage of ischemic stroke treated in the inpatient department of the First Affiliated Ho SPital of Guangxi University of Chinese Medicine from March 2020 to December2020,according to the standard of traditional Chinese medicine diagnosis and curative effect evaluation of apoplexy,select the syndromes of patients with four syndrome types:wind-phlegm with blood-stasis syndrome,phlegm-heat and fu-solid syndrome,Qi deficiency with blood-stasis syndrome and Yin deficiency and wind movement syndrome for the spletetudy.All patients in each group completed the National Institute of Health Stroke Scale(NIHSS)within 24 hours of admission to evaluate the neurological deficit.Fasting venous blood was drawn in the early morning of the next day,and the expression levels of serum VIP and SP in each group were detected by enzyme linked immunosorbent assay(ELISA).At the same time,middle-aged and elderly people who meet the enrollment criteria in our hospital’s health examination center were selected as the control group.Collect the relevant clinical data of the control group,and draw fasting venous blood from the control group in the early morning of the next day,and perform the detection of serum VIP and SP.Statistical analysis was performed on the result data of each group using SPSS 25.0 software.Results:(1)The serum VIP expression level of patients with acute ischemic stroke was higher than that of the control group,and the difference was statistically significant(P<0.05);the serum SP expression level was lower than that of the control group,and the difference was statistically significant(P<0.05).(2)Comparison of serum VIP and SP expression levels in each group of patients in acute ischemic stroke:(1)The level of serum VIP expression increased in the order of Yin deficiency and wind movement syndrome→Qi deficiency and blood stasis syndrome→wind-phlegm and blood-stasis syndrome→phlegm-heat and fu-solid syndrome.Comparison of VIP expression levels of 4 syndrome types: The syndrome of phlegm-heat and fu-solid was significantly higher than that of wind-phlegm and blood stasis、Qi deficiency and blood stasis、yin deficiency and wind movement,and the differences were statistically significant(P<0.05).The syndrome of wind-phlegm and blood stasis was significantly higher than that of Qi deficiency and blood stasis、Yin deficiency and wind movement,and the difference was statistically significant(P<0.05).The syndrome of qi deficiency and blood stasis was higher than that of Yin deficiency and wind movement,but the result were not statistically significant(P>0.05).(2)The level of serum SP expression reduced in the order of Yin deficiency and wind movement syndrome→Qi deficiency and blood stasis syndrome→wind-phlegm and blood-stasis syndrome→phlegm-heat and fu-solid syndrome.Comparison of SP expression levels of 4 syndrome types:The syndrome of phlegm-heat and fu-solid was significantly lower than that of wind-phlegm and blood stasis 、 Qi deficiency and blood stasis 、 Yin deficiency and wind movement,and the differences were statistically significant(P<0.05).The syndrome of wind-phlegm and blood stasis was significantly lower than that of Qi deficiency and blood stasis、Yin deficiency and wind movement,and the difference was statistically significant(P<0.05).The syndrome of Qi deficiency and blood stasis was lower than that of Yin deficiency and wind movement,but the result were not statistically significant(P>0.05).(3)In the acute phase of Ischemic stroke,the NIHSS scores of patients was highest among phlegm-heat and fu-solid syndrome,followed by the group of wind-phlegm and blood-stasis syndrome.The order of the NIHSS score of the patients in each group is:phlegm-heat and fu-solid syndrome>wind-phlegm and blood-stasis syndrome>Yin deficiency and wind movement>Qi deficiency and blood stasis syndrome.Comparison of NIHSS scores among different syndrome groups:the syndrome of phlegm-heat and fu-organs and wind-phlegm-stasis syndrome were significantly higher than those of Qi deficiency and blood-stasis syndrome,there were a significant difference(P<0.05).Conclusion:The expression levels of serum brain gut peptides VIP and SP in patients with acute ischemic stroke are obviously abnormal,and there are certain differences among different TCM syndrome types.The abnormal expression levels of the two indicators are especially obvious in the phlegm-heat and fu-solid syndrome,followed by wind-phlegm and blood-stasis syndrome.The abnormal expression levels of VIP and SP reflect the syndrome status of phlegm-heat and fu-solid 、 phlegm-blood stasis in patients with acute ischemic stroke.It is suggested that the abnormal expression of VIP and SP in the acute phase of ischemic stroke is closely related to the interaction of Phlegm、blood-stasisand and heat.VIP and SP are expected to become the objective indicators of syndrome differentiation in the acute phase of ischemic stroke.
