| Objective: The purpose of this study is to observe the clinical efficacy and safety of five-element acupuncture therapy in the treatment of soil walking patients with diarrhea type irritable bowel syndrome(spleen and kidney Yang deficiency type),and to verify and explore its clinical practical value.To provide a clinical basis for the literature research and clinical application of ancient acupuncture and moxibustion.Methods: Ninety cases of soil pedestrians in line with diarrhea type irritable bowel syndrome(spleen-kidney Yang deficiency type)were included and randomly divided into three groups according to random number table method,in which the five elements acupuncture therapy was used as the treatment group,the traditional Chinese medicine control group was used as the traditional Chinese medicine group,and the western medicine control group was used as the western medicine group,with 30 patients in the three groups respectively.In the treatment group,five elements of acupuncture and moxibustion were given.Acupoints were selected for the first treatment of patients in the group,bilateral Feishu,Perico baoshu,Xinshu,Ganshu,Pishu,Shenshu,Chongyang and Taibai.After the first treatment for each patient,the original acupoints(bilateral Chongyang and Taibai acupoints)were selected for the second to fourth treatment,once a week for 4 weeks.In the traditional Chinese medicine group,the decoction of Fuzi Lizhong Decoction and Sishen Pill combined formula were dialectically added or subtracted,one dose a day,water decocted 400 ml,twice a day,20 minutes before breakfast and dinner,for a course of 4 weeks.The western medicine group was treated by taking pivironium bromide tablets,50 mg each time,3 times a day,after meals,for 4weeks.All patients were followed up for another 4 weeks,and the total duration of the clinical study was 8 weeks.Patients choose of three groups of different treatment methods before treatment and after treatment of 4 weeks are separately symptom integral evaluation,syndromes of traditional Chinese medicine efficacy evaluation,Bristol total credit assessment,IBS life quality evaluation,IBS symptoms severity score evaluation,Hamilton depression rating scale,channels and collaterals determination of infrared thermal imaging of the skin temperature and the recurrence rate,thus to assess differences in clinical effect and safety of three groups of patients.The research data of the whole project were summarized by Excel,sorted out by SPSS25.0 statistical data processing software,and analyzed and processed statistically.Results:(1)The rapeutic effect of TCM syndromes: The total effective rate of the treatment group was 89.7%,and the total effective rate of the Chinese medicine group was 80.0%,and the total effective rate of the western medicine group was 65.5%.There was a significant difference in the efficacy of the three groups(P<0.05),indicating that the therapeutic effect of the treatment group was significant.(2)Total scores of TCM symptoms: Compared with before treatment,the total scores of TCM symptoms in the three groups were significantly improved,with statistical significance(P<0.05).The improvement effect of the treatment group was significantly better than that of the TCM group and the western medicine group(P<0.05),and the improvement effect of the Chinese medicine group was significantly better than that of the western medicine group(P<0.05).(3)The score of IBS-SSS symptom severity in both the treatment group and the TCM group was significantly lower than before treatment(P<0.05),the difference was statistically significant.After treatment,the decrease in the treatment group was more obvious than that in the TCM group,the difference was statistically significant(P<0.05).(4)IBS-QOL quality of life score: After treatment,IBS-QOL quality of life score in daily exercise and mental state of the three groups were decreased compared with before treatment(P<0.05),and the improvement degree in the treatment group and the Chinese medicine group was significantly higher than that in the western medicine group(P<0.05).In terms of daily social interaction,emotional status,work influence,energy change and diet influence,there was statistical significance between the treatment group and the western medicine group(P<0.05),and there was also statistical significance between the Chinese medicine group and the western medicine group(P<0.05).In terms of sleep state,there was statistical significance between the treatment group and the traditional Chinese medicine group and the western medicine group(P<0.05).(5)HAMD score: HAMD score of the three groups was significantly decreased after treatment(P<0.05).Among the three groups of patients,the treatment group and the Chinese medicine group had more obvious curative effect,which was better than the western medicine group(P<0.05).(6)Total score of Bristol stool traits: After treatment,the total score of Bristol stool traits in the three groups was significantly lower than that before treatment,the difference was statistically significant(P<0.05),and the decrease was more significant in the treatment group(P<0.05),indicating that the efficacy of Five-element acupuncture therapy was better than that of the traditional Chinese medicine group and the western medicine group.(7)The skin temperature of infrared thermal imaging of meridian and meridian: There was no significant difference in the average temperature of former Du Ermai,Pishu and Shenshu among the three groups,with no statistical significance(P>0.05).After treatment,the skin temperature of Pishu,Shenshu and Rendu two veins in treatment group and Chinese medicine group were significantly increased,and that in treatment group was better than that in Chinese medicine group(P<0.05).After treatment,there was no significant difference in skin temperature between the western medicine group before and after treatment(P>0.05).(8)Comparison of recurrence rate: The recurrence rate was 20.7% in the treatment group,30.0% in the traditional Chinese medicine group and 55.2%in the western medicine group after 4 weeks of follow-up after the end of treatment,with statistical significance(P<0.05).It is suggested that the treatment group can control the disease effectively and its clinical effect is more stable.(9)Safety comparison: No special discomfort or adverse reactions occurred in the three groups during the treatment.There were no obvious abnormal changes in vital signs and auxiliary examinations in the three groups.Conclusion(s):(1)The five elements acupuncture therapy improved the main TCM syndromes and overall efficacy of IBS-D patients with spleen-kidney Yang deficiency,which were better than those of the traditional Chinese medicine group and the western medicine group.In addition,the five elements acupuncture therapy has a significant effect on improving patients’ quality of life,anxiety and depression,stool characteristics and the severity of IBS symptoms.It has certain clinical significance and is worth popularizing and applying.(2)The combination of infrared skin temperature detection therapy provides a new idea and method for the clinical diagnosis and treatment of other diseases,so as to further guide clinical practice.(3)No relevant serious adverse reactions occurred in the study of this subject,which confirmed the safety and effectiveness of Five-element acupuncture therapy and can be popularised in clinical practice. |
