Objective:In this experiment,by observing the morphological changes of the neck muscles in rabbits with cervical disc degeneration,the integrin α5,β1 fibronectin(Fn)in the nucleus pulposus of the intervertebral disc,and the blood interleukin 6(IL-6)content changes,it is clear that the mechanical changes are in the disc The mechanism of action in the process of degeneration provides experimental support for the clinical treatment of Cervical Intervertebral Disc DegenerationMethod:According to the random number table,24 New Zealand rabbits were divided into blank group,model group,and needle knife group,with 8 rabbits in each group.The blank group,model group and needle knife group are fixed in the same way.The method of "low head position plus wind,cold and damp stimulation" was used to establish rabbit cervical intervertebral discs induced by abnormal stress.The degeneration model is verified by cervical magnetic resonance(MRI)imaging to verify the success of the model.The needle-knife group intervened one week after the completion of model building.The intervention was conducted in a sterile state with the four-step acupotomy as a reference,once a week,for a total of 4 weeks of treatment.After the end,HE staining method was used to observe the morphological changes of the neck muscle tissue of each group of experimental rabbits;Western Blot method and Real-time PCR method were used to detect the changes of integrin α5,β1 and Fn content in the intervertebral disc nucleus pulposus;Elisa method was used to detect blood IL The content of-6 changes.Results:1.General observation:New Zealand rabbits in the blank group are in good spirits,have normal appetites,stools are dark brown balls with a slightly hard texture,no significant changes in body weight,soft muscles under neck palpation,no indurations or cords,and no obvious resistance when grasping;Compared with the blank group,model rabbits can occasionally lose appetite,curl up behind the cage,occasionally bite the cage,some may sneeze,head and neck tilted to one side but they can recover spontaneously,and sometimes the size of defecation is not uniform or slightly Unformed,even mushy stool,large struggling before and after modeling,may have hurting behavior,unfavorable neck movement,stiffness of the lower neck muscles on palpation,and formation of a cord;the rabbits in the needle knife group are in normal spirit and are treated There was good coordination at the time and no violent struggle.Compared with the model group,the appetite was good,the weight increased slightly,and the muscle softness was improved on palpation.2..Cervical MRI results showed that the nucleus pulposus in the blank group showed a high signal state on the sagittal plane,the disc height was normal,and the cross-section showed that the signal in the nucleus pulposus was uniform and the border with the annulus fibrosus was clear;compared with the blank group,the discs in the model group There was a slight decrease in height.Some of the intervertebral disc nucleus pulposus seen in the sagittal plane was darker and gray,and the signal intensity was significantly lower than that of the blank group.Compared with the uniform distribution of the nucleus pulposus seen in the blank group’s intervertebral disc cross-section,the model group’s nucleus pulposus The structure is obviously unevenly distributed,and the boundary between the annulus fibrosus and the nucleus pulposus is blurred and difficult to distinguish.3.HE staining showed that the muscle fibers in the blank group were clearly structured and arranged in parallel in long strips.The muscle nuclei were scattered near the fibers,and the muscle cells had no deformation and edema.Compared with the blank group,the muscle fibers in the model group appeared thick and thin without shrinkage or deformation.Muscle nuclei are clustered nearby,sarcomeres are not clear,and inflammatory infiltrates are scattered.Compared with the model group,muscle fiber contractures are improved in the acupotomy group,muscle nuclei are distributed regularly,and inflammatory cell infiltration is significantly reduced.4.The results of Western Blot method showed that compared with the blank group,the expression of integrin α5,β1 and Fn protein in the intervertebral disc nucleus pulposus of the model group was significantly reduced(P<0.01);compared with the model group,the acupotomy group integrin α5 The protein content of β1,β1 and Fn increased significantly(P<0.01).5.Real-time PCR results showed that:compared with the blank group,the relative expression of integrin α5,β1,and Fn mRNA in the intervertebral disc nucleus pulposus of the model group were significantly reduced(P<0.01);compared with the model group,acupotomy In the group,integrin α5mRNA was significantly increased(P<0.01),the relative expression ofβ1 mRNA was not statistically significant(P>0.05),and Fn mRNA was significantly increased(P<0.05).6.The ELISA method showed:Compared with the blank group,the relative expression of IL-6 in the blood of the model group was significantly increased(P<0.01);compared with the model group,the IL-6 in the blood of the needle knife group was significantly decreased(P<0.01).Conclusion:1.It can be seen from the results of cervical MRI that the "head down position plus wind,cold and damp stimulation" model selected in this experiment is feasible.2.The needle knife can repair the disordered arrangement of muscles and promote tissue repair.3.The possible mechanism of acupotomy to delay cervical disc degeneration is to adjust the expression levels of integrins α5,β1 and Fn through mechanical acupuncture,and reduce the release of inflammatory factor IL-6,so as to achieve the purpose of delaying cervical disc degeneration..
