Clinical Effect And Mechanism Of Qingchen Mixture In Treating Senile Chronic Functional Constipation With Intestinal Qi Stagnation Syndrome

Objective:Through further research on the mechanism of Qingchen Mixture in our hospital,combined with the idea of multi-target effect of traditional Chinese medicine prescription,and using modern medical detection means,this paper discusses its curative effect on Chronic functional constipation(CFC)in the elderly with intestinal qi stagnation syndrome and its effect on serum stem cell factor(SCF),substance P(SP),5-hydroxytryptamine(5-HT)and Vasoactive intestinal peptide(VIP).To further explore the action pathway of Qingchen Mixture,and provide theoretical support for clinical application of Qingchen Mixture in the treatment of CFC.Methods:From April 2019 to December 2020,90 patients with CFC over 60 years old were selected and divided into three treatment groups according to the principle of random,parallel and controlled grouping.The treatment course of the three treatment groups was 7 days.the single drug group took lactulose oral solution(Beijing Hanmei Pharmaceutical Co.,Ltd.,100ml/bottle)10ml,2 days;In the combined group of Chinese medicine,lactulose oral solution(the same manufacturer as above)10ml,2/day,and Qingchen Mixture 20ml 3/day;The western medicine combined group was given lactulose oral solution(same manufacturer as above)10ml,2 days,and orally taken mosapride citrate dispersible tablets(Chengdu Kanghong Pharmaceutical Co.,Ltd.,5mg*20 tablets),1 tablet,3 days.Before and after treatment,the peripheral blood was collected,and the changes of constipation symptom scores before and after treatment were recorded and analyzed,and the changes of SCF,SP,5-HT and VIP in peripheral blood before and after treatment were compared.Results:After treatment,the total effective rate was 85% in single drug group,91.30% in traditional Chinese medicine group and 93.76% in western medicine group.The TCM syndrome scores of the three groups all decreased,and there was no statistical significance among the three groups(P>0.05).After treatment,there were significant differences in the total scores of constipation symptoms among the three groups compared with those before treatment(P<0.05).At the same time,after treatment,there were significant differences in the total scores of constipation symptoms in the combined Chinese medicine group compared with the single medicine group and the combined western medicine group(P<0.05).There were significant differences in defecation difficulty,defecation time,anal distention,falling feeling,incomplete defecation,abdominal distention and bloating between the single drug group and the traditional Chinese medicine combined group(P<0.05).After treatment,there were statistical differences between Chinese medicine combined group and western medicine combined group in the symptoms of defecation difficulty,abdominal distension and bloating(P<0.05).After treatment,the SCF concentrations of the three groups were statistically significant compared with those before treatment(P<0.05),and after treatment,the SCF concentrations in the combined Chinese medicine group were significantly different from those in the single medicine group(P<0.05).After treatment,there was no significant difference in 5-HT concentration between the three groups before and after treatment(P>0.05).Compared with the SP concentration before treatment,the SP concentration in the three groups was significantly different(P<0.05).Compared with the VIP concentration before treatment,the VIP concentration of the three groups had statistical differences(P<0.05).Conclusion:Qingchen Mixture combined with lactulose can obviously improve the clinical symptoms of constipation,and has a remarkable curative effect on functional constipation in the elderly with qi stagnation syndrome.Compared with lactulose,it is more effective in improving the symptoms of defecation difficulty,defecation time,anal distention,falling feeling,incomplete defecation,abdominal distension and bloating,and it is more effective than lactulose combined with mosapride.Qingchen Mixture combined with lactulose can promote SCF expression,up-regulate excitatory neurotransmitter P,down-regulate inhibitory neurotransmitter VIP secretion,and improve constipation symptoms of patients.At the same time,Qingchen Mixture combined with lactulose can promote SCF expression better than lactulose alone.5-HT did not get a meaningful conclusion in this experiment.
