Research purposeData mining is used to study the distribution of syndrome types,pathogenesis characteristics and medication rules of anal pain,so as to provide reference for clinical treatment of anal pain.The research methodsIn this study,the medical cases and words and literatures related to the treatment of anal pain by traditional Chinese medicine in China Journal Fulltext Database(CNKI)and Chinese Medical Book(Fifth Edition)were collected,and the prescription for internal administration was screened out,and the statistical table of drugs and syndrome type,efficacy,four natures and five flavors of drugs,and meridian return database were established respectively.SPSS25.0 was used to carry out frequency statistics on the frequency of drug association in the database,and then cluster analysis and association rule analysis were carried out on the high-frequency drugs in combination with SPSmodeler18.0.Finally,complex network diagram was constructed through Cytoscape to intuitively reflect drug association rules.Results of the studyIn this study,164 literatures(62 ancient literatures and 102 modern literatures)were included,404 prescriptions(89 ancient prescriptions and 315 modern prescriptions)were included,and 274 kinds of drugs were involved(11 kinds of unknown drugs were excluded).1、The type of distributionIn this study,the 404 rules were subdivided into 107 cases of spleen-stomach asthenia,91 cases of blood-stasis syndrome,69 cases of intestinal dampness-heat,64 cases of stagnation of liver qi,54 cases of deficiency of the liver and kidney,17 cases of middle focal block,and 2 cases of chalice stagnation sputum interference syndrome(the frequency was too small to be included in the discussion).2、Pharmaceutical analysis(1)Drug frequency:This study involved 263 drugs,among which 50 drugs were used with>frequency 20 times.Drugs with a frequency of ≥10 times were selected for each syndrome type(drugs with a frequency of ≥4 times for middle focal block),which were 25 spleen-stomach asthenia,30 flavors of blood-stasis syndrome,20 flavors of intestinal dampness-heat,17 flavors of stagnation of liver qi,24 flavors of deficiency of the liver and kidney,and 10 flavors of middle focal block.(2)Drug efficacy:Among the commonly used drugs(>20 times)screened out in the original database,the most common drugs for deficiency-nourishing were 36.13%,followed by drugs for blood-activating and stasis-eliminating(13.22%)and drugs for regulating qi(12.31%),accounting for 58.00%in total.The frequency of drug use in different syndrome types was as follows:①Spleen-stomach asthenia:deficiency-nourishing drugs(57.56%),treating exterior syndromes drugs(15.64%),regulating qi drugs(9.36%);② blood-stasis syndrome:blood-activating and stasis-eliminating drugs(31.48%),deficiency-nourishing drugs(25.18%),regulating qi drugs(13.68%);③intestinal dampness-heat:deficiency-nourishing drugs(22.96%),antipyretic drugs(16.09%),inhibiting-damp drugs(15.57%);④stagnation of liver qi:deficiency-nourishing drugs(44.10%),regulating qi drugs(25.17%),treating exterior syndromes drugs(15.14%);⑤deficiency of the liver and kidney:deficiency-nourishing drugs(52.34%),treating exterior syndromes drugs drugs(12.67%),inhibiting-damp drugs drugs(9.37%);⑥middle focal block:regulating qi drugs(26.76%),dampness-transforming drugs(25.35%),deficiency-nourishing drugs(12.68%).(3)Sexual flavour and meridian tropism for drugs:The distribution of commonly used drug flavors in the original database was warm drugs(43.24%),cold drugs(31.01%),neutral nature drugs(22.15%),sweet drugs(31.34%),bitter drugs(29.43%)and pungent drugs(28.58%)in the order of frequency of use,among which sweet and warm drugs ranked first.The drugs were mainly transferred to the spleen(22.86%),liver(16.68%),lung(13.30%)and heart(13.29%).The distribution of different syndromes was as follows:①The distribution of drug flavor in spleen-stomach asthenia was warm drugs(55.51%),neutral nature drugs(20.38%),cold drugs(18.08%),sweet drugs(44.36%),bitter drugs(27.03%),and pungent drugs(22.18%),in order of frequency of use,among which sweet and warm drugs took the first place;The drugs mainly entered the spleen,lung and stomach meridians,mostly in the spleen meridian(28.85%).②The distribution of drug flavor in blood-stasis syndrome was warm drugs(45.40%),cold drugs(31.84%),neutral nature drugs(22.76%),bitter drugs(33.87%),pungent drugs(32.26%),sweet drugs(25.87%),in the order of frequency of use,among which bitter and warm drugs took the first place.The drugs mainly entered the liver,spleen and heart meridians,mostly in the liver meridian(23.79%).③The distribution of drug flavor in intestinal dampness-heat was cold drugs(40.11%),warm drugs(33.25%),bitter drugs(34.49%),sweet drugs(26.78%)and pungent drugs(23.45%),in the order of use frequency,among which bitter and cold drugs took the first place.The drugs were mainly transferred to the spleen,stomach,liver and large intestine,mostly to the spleen meridian(20.00%).④The distribution of drug flavor in stagnation of liver qi was cold drugs(38.08%),warm drugs(35.86%),neutral nature drugs(26.06%),bitter drugs(30.56%),pungent drugs(27.63%)and sweet drugs(27.14%),in the order of frequency of use,among which bitter and cold drugs took the first place.The drugs were mainly transferred to the spleen(27.92%),lung(19.46%)and liver(15.85%).⑤The distribution of drug flavor in deficiency of the liver and kidney was warm drugs(53.44%),cold drugs(23.42%),neutral nature drugs(23.14%),sweet drugs(43.19%),pungent drugs(22.52%),bitter drugs(16.97%),in the order of frequency of use,among which sweet and warm drugs took the first place.The drugs were mainly transferred to liver(19.88%),spleen(17.48%)and kidney(17.27%).⑥The distribution of drug flavor in middle focal block was warm drugs(59.15%),neutral nature drugs(16.90%),pungent drugs(40.60%),bitter drugs(31.58%),sweet drugs(15.79%),in order of frequency of use,among which pungent and warm drug took the first place.The drugs were mainly transferred to spleen(32.1 3%),lung(20.81%)and stomach(19.46%).(4)Clustering analysisThe drug clustering of each syndrome type was as follows:9 categories of spleen-stomach asthenia,5 categories of blood-stasis syndrome,4 categories of intestinal dampness-heat,5 categories of stagnation of liver qi,5 categories of deficiency of the liver and kidney,and 2 middle focal block.(5)Correlation rule analysisThe results of drug Correlation rules for each syndrome type were as follows:common drug use in 7 and 8 groups for spleen-stomach asthenia.The drug combinations with the most frequent use were atractylodes macrocephala-dried tangerine peel(49)and astragalus-angelica-atractylodes macrocephala(45).Common drug use in 5 and 6 groups for blood-stasis syndrome.The most frequent drug combinations were peach kernel-safflower(54),ligusticum chuanxiong-safflower(54),peach kernel-safflower-licorice(4 7).Common drug use in 2 and 10 groups for intestinal dampness-heat.The drug combinations with the most frequent use were phellodendri chinensis-alisma alisma(25)and angelica-chinese atractylodes-phellodendri chinensis(19).There were 5 and 11 groups commonly used for stagnation of liver qi.The drug combinations with the most frequent use were licorice-angelica sinensis(41)and licorice-white paeonia-bupleurum(37).5 and 6 groups were commonly used for deficiency of the liver and kidney.The drug combinations with the most frequent use were angelica-prepared radix rehmanniae(27)and prepared radix rehmanniae-alisma alisma-angelica(17).The commonly used drugs for middle focal block were in 4 and 2 groups.The most frequently used drugs were dried tangerine peel-atractylodes tractylodes(8),cortex magnoliae officinalis-ginseng-baked ginger(5),dried tangerine peel-atractylodes tractylodes-cortex magnoliae officinalis(5).(6)Complex network analysisThe core drugs and the core drug pairs of each syndrome type were as follows:spleen-stomach asthenia:licorice,astragalus membranaceus,licorice-astragalus membranaceus;blood-stasis syndrome:peach kernel,safflower,chuanxiong,licorice,peach kernel-safflower,licorice-chuanxiong,peach kernel-licorice;intestinal dampness-heat:alisma nissima,phellodendron chinense,alisma nissima-phellodendron chinense;stagnation of liver qi:licorice,bupleurum,licorice-bupleurum;deficiency of the liver and kidney:angelica,prepared aadix rehmanniae,angelica-prepared aadix rehmanniae;middle focal block:dried tangerine peel,dried tangerine peel-magnolia officinalis,dried tangerine peel-atractylodes.Research Conclusion(1)This disease is a syndrome of deficiency and excess.The treatment of spleen deficiency is based on invigorating spleen,supplemented by clearing heat and dampness,promoting blood circulation and removing blood stasis,regulating qi and eliminating cold and relieving pain.While replenishing spleen qi and blood,qi should be kept in mind to prevent tonifying drugs from filling and obstructing stomach.(2)The use of drugs mainly enters the spleen meridian and is closely related to Qi and blood.(3)dampness and spleen disease influence each other.
