ObjectiveTo evaluated the efficacies of the antiemetic efficacies of standard antiemetic regimen with/without Sihua acupoint catgut-embedding in patients with breast cancer using Highly or moderately emetogenic chemotherapy(HEC/MEC).To observe the influence of sihua acupiont on the quality of life and to demonstrate the simplicity,safety and long-term efficacy of catgut-embedding.MethodsThrough a open,prospective,randomized,two-stage cross-over design controlled trial,according to the random digital table,32 patients with Highly or moderately emetogenic chemotherapy(HEC/MEC)were randomly divided into two group,each with 16 cases.The experiment was divided into two stages which were equal to two routines of chemotherapy.The test plans were A and B.Regimen A was the combination of triple antiemetic drugs with Sihua acupoint catgut-embedding while regimen B was the combination of triple antiemetic drugs without Sihua acupoint catgut-embedding,as a blank control group.All the treatment was performed in the first day of chemotherapy.The first stage,the first group was treated with A regimen,the second group.was treated with B regimen.After the interval of 20 days,the second stage was started.The design was the opposite of the previous design,which meant,the treatment order of the first group was the first A regimen followed by the B regimen;the treatment order of the second group was the first B regimen followed by the A regimen.To recorded the degree of nausea and vomiting during the first seven days after chemotherapy,and the quality of life scored by the MOS item short from health survey(SF-36),blood routine examination before and after treatment,and to compared the changes in the various indicators before and after each stage of treatment;according to WHO on the adverse reaction of chemotherapy.SPSS 22.0 statistical software was used to analyze the index data of the subjects,so as to evaluate the difference in the efficacy of Sihua acupoint catgut-embedding on Chemotherapy-induced nausea and vomiting and its influence on the quality of life in breast cancer patients.ResultsA total of 30 subjects completed the study.Before the treatment,there was no statistical difference in age,height,weight,operation or not,disease,KPS score,chemotherapy frequency,blood routine examination and the quality of life items between the two group(P>0.05).The two groups had different digestive tract toxic reactions of nausea and vomiting after treatment with regimen A and B.The daily grade of nausea and vomiting symptoms and the occurrence days of the two symptoms after chemotherapy were compared between the two groups.there was no significant difference in the grade and occurrence days of vomiting symptoms in both group in terms of efficacy of vomiting(P>0.05).In terms of the efficacy of nausea symptoms,there was statistically significant different regimen A and regimen B in the daily symptom grade except the 2nd day in the first group,all the 7 days in the second group(P<0.05).There were significant differences in the days of symptom onset between the two groups(P<0.05).In addition,regimen A had lower symptom grade,fewer days of the symptom occurrence and higher complete remission rate(the 1st group:regimen A:73.33%;regimen B:46.67%;the 2nd group:regimen B:46.67%;regimen A:80%).Through the analysis of generalized estimation equations,the measurement analysis of different order,stage,regimen and time was repeated.The generalized estimation equation was used to It was found that there were significant differences in the nausea and vomiting symptom grades between different programs(P<0.05),and the nausea and vomiting symptom grades of regimen A were lower than that of regimen B,indicating the superior efficacy of regimen A.There were significant differences in the severity of nausea and vomiting symptoms at different time points(P<0.05),and the risk of vomiting was the highest in the first three days,and the risk of nausea symptoms was higher in the first six days after chemotherapy than in the seventh day.However,there was no statistical difference in the risk of nausea and vomiting in different sequence and different treatment stages(P>0.05).In terms of the quality of life,there was no statistically significant change in the scores of the 1st group before and after regimen A in the first stage while scores of the 2nd group were decreased in the terms of general health,Physical Functioning,Role-Physical,Bodily Pain,Vitality,Social Functioning,Role-Emotional and Mental Health to varying degrees(P<0.05).In the second stage,the scores of SF-36 of the 1st group before and after regimen B in general health,Physical Functioning,Role-Physical,Bodily Pain,Vitality,Social Functioning and Mental Health were scale to some extent decrease(P<0.05),while in the 2nd group the scores in general health,Physical Functioning,Bodily Pain,Role-Emotional and Mental Health were statistically significantly increased after regimen A(P<0.05).The results showed that regimen A significantly improved the quality of patients in the items of general health status,physiological function,physical pain,emotional function and mental health compared with regimen B.There was no catgut-embedding related adverse events in both groups.ConclusionEven under the triple antiemetics prophylaxis,Chemotherapy-induced nausea and vomiting still occurred within 7 days in varies degrees.Sihua acupoint catgut-embedding can play an obvious role in the prevention and treatment of nausea caused by chemotherapy on the basis of triple antiemetic prophylaxis by reducing the days of nausea and the grade of nausea,and can prevent vomiting.It can improve the quality of life of patients from general health,physiological function,emotional function,mental health and other aspects.It has the convenience of acupoints’ selection,time,continuous efficacy and safety.
