Objective:Aimed to clarify the link between PFO and ischemic stroke,and provide data for clinical practice,in this study,we compared the clinical and imaging features in ischemic stroke patients 18 to 55 years with and without patent foramen ovale(PFO).The relationship between PFO and ischemic stroke was analyzed,and the clinical and imaging characteristics of ischemic stroke patients with PFO was documented.Method:328 patients 18 to 55 years old who were diagnosed as an acute ischemic stroke from January 2018 to February 2021 were included.The patients were divided into PFO(+)group and PFO(-)group according with or without PFO.The PFO(+)group is further divided into a small amount of shunt,a medium amount of shunt and a large amount of shunt according to the shunt volume.The general clinical data of the two groups of patients including age,gender,risk factors for cerebrovascular diseases(hypertension,diabetes,smoking history,drinking history,family history of stroke),National Institutes of Health Stroke Scale at the time of hospitalization and discharge,laboratory examinations,and imaging data were collected.General clinical data,NIHSS score,cerebrovascular disease risk factors,imaging characteristics(number of lesions,location,size,and distribution area of responsible blood vessels for infarcts)were compared between PFO(+)group and PFO(-)group,and the influence of different shunt volume on the degree of neurological deficit and imaging characteristics was analyzed.Finally,the characteristics of the risk factors of young patients with cerebral infarction and the role of PFO in them are obtained,which can provide help for the early clinical identification of PFO-related strokes.Result:1.A total of 328 patients was enrolled in this study,There were 135 cases in PFO(+),and 193 cases in PFO(-)group.The incidence of PFO is about 41%;There were 100 males(n=100,74.1%)and 35 females(n=35,25.9%)in PFO(+)group,163 males(n=163,84.5%)and 30 females(n=30,15.5%)in the PFO(-)group.The proportion of male patients in both groups was higher than that of female patients,and statistical analysis showed that the difference was statistically significant(χ~2=5.388,P=0.02).The mean age of patients in PFO(+)group was 46.47±7.79 years,and the mean age of patients in PFO(-)group was46.09±7.87 years,There was no statistically significant difference in the age between the two groups(P>0.05).2.22 cases(16.3%)of hyperhomocysteinemia in the PFO(+)group,50cases(25.9%)of hyperhomocysteinemia in the PFO(-)group,the difference of homocysteinemia proportion between two groups was statistically significant(P<0.05);The proportion of hypertension,diabetes,hyperlipidemia,smoking,drinking,past stroke history,and the proportion of atrial fibrillation is lower in PFO(+)group than that of PFO(-)group,but the difference was not statistically significant(P>0.05).3.The NIHSS scores on admission and discharge between PFO(+)and PFO(-)groups were compared,there was no difference(P>0.05).And in PFO(+)group,there was no difference on NIHSS scores among the different amount of shunt groups(P>0.05).4.Analysis of imaging results between the two groups was done.There was no statistically significant difference in the distribution of infarcts between PFO(+)and PFO(-)groups(P>0.05).and there was no statistically significant difference in the distribution of infarcts among the different amount of shunt groups(P>0.05).Conclusions:1.The incidence of PFO in ischemic stroke was high,and there were relatively few cerebrovascular risk factors such as hypertension,diabetes,and hyperlipidemia in ischemic stroke patients combined with PFO,PFO may play a role in the onset of ischemic stroke.2.There was no significant correlation between the PFO and it’s shunt volume to the imaging characteristics of infarcttion or the neurological deficit values in ischemic stroke patients.
