Objective:Research and analyze the correlation between exhaled nitric oxide,tidal lung function and allergens,and explore its clinical value in infantile wheezing diseases.Methods:A total of 180 infants and young children aged 3 and under who were hospitalized with wheezing disease in the Pediatric Respiratory Department of Xiangxi Autonomous Prefecture People’s Hospital from November 2018 to October 2019 were selected as the observation group.Among them,130 cases were diagnosed as wheezing pneumonia,bronchial There were 50 cases of asthma,and 101 cases of children diagnosed with bronchial pneumonia were selected as the control group during the same period.All children were tested for FeNO,tidal breathing pulmonary function and allergen.Results:(1)In infants<1 year old,the positive rate of tree pollen in inhalation allergens was the highest in each group,the positive rate of milk in edibility allergens was the highest in asthmatic pneumonia and bronchopneumonia group,the positive rate of beef in bronchial asthma group was the highest,and milk was the second.During the period of 1-3 years old,the positive rate of house dust mites in the inhalation allergen group was the highest,the positive rate of milk in the edibility allergen group was the highest,followed by egg white.(2)There was no statistically significant difference in the positive rate of inhaled allergens between the bronchopneumonia group,wheezing pneumonia group,and bronchial asthma group between<1 year old and 1-3 years old(P>0.05).In the asthmatic pneumonia group,there was a statistically significant difference in the positive rate of egg white among the ingested allergens(c(17)=9.57,P=0.002).The positive rate of beef in ingestion allergen of bronchial asthma group was statistically different(P<0.05).(3)There was no statistically significant difference in the positive rate of inhaled allergens among the bronchopneumonia group,wheezing pneumonia group and bronchial asthma group(P>0.05).The positive rate of egg white was different among the three groups in the positive rate of ingested allergens.There is statistical significance(c(17)=13.70,P=0.0011).(4)The FeNO values of the bronchopneumonia group,asthmatic pneumonia group and bronchial asthma group were 6.0(5.0,8.0)ppb,8.0(6.0,14.0)ppb,and 11.5(7.0,21.0)ppb,and there were statistical differences in the FeNO values among the three groups(P<0.05).(5)The levels of TPTEF/TE in bronchial pneumonia group,asthmatic pneumonia group and bronchial asthma group were(23.9±5.6)%,(21.8±4.7)%and(21.2±4.4)%,and there was statistical difference in the levels of TPTEF/TE among the three groups(P<0.05).The levels of VPEF/VE in bronchial pneumonia group,asthmatic pneumonia group and bronchial asthma group were(23.9±5.7)%,(22.2±4.4)%and(21.5±4.3)%,and there was statistical difference in the levels of VPEF/VE among the three groups(P<0.05).(6)There was no correlation between FeNO and TPTEF/TE and VPEF/VE in bronchopneumonia group,asthmatic pneumonia group and asthma group(r=0.050,r=-0.03,r=0.24,r=0.045,r=-0.031,r=0.201,P>0.05).(7)There was no correlation between FeNO and t Ig E,house dust mite s Ig E and milk s Ig E in bronchopneumonia group(r=0.073,r=0.079,r=0.045,P>0.05),but there was a positive correlation between FeNO and egg white s Ig E(r=0.204,P<0.05).There was no correlation between FeNO and t Ig E,house dust mites,milk and egg white s Ig E in the asthmatic pneumonia group(r=0.108,r=0.048,r=-0.080,r=-0.127,P>0.05).There was no correlation between FeNO and house dust mites and milk s Ig E in the bronchial asthma group,In the bronchial asthma group,FeNO was not correlated with house dust mites and milk s Ig E(r=0.062,r=0.125,P>0.05),but was positively correlated with t Ig E and egg white s Ig E(r=0.392,r=0.443,P<0.05).Conclusion:(1)Allergens are distributed differently in different age groups of wheezing infants,but house dust mites,milk,and egg white are still the three most common allergens that cause infants and young children to wheeze.(2)FeNO can reflect the airway eosinophilic inflammation level to some extent in infantile wheezing diseases.(3)In infants with wheezing diseases,TPTEF/TE and VPEF/VE can be used as reference indicators to reflect airway ventilation function,but they have no correlation with FeNO levels.(4)There is a positive correlation between serum t Ig E and FeNO levels in children with bronchial asthma,so serum t Ig E can reflect the degree of airway eosinophilic inflammation to a certain extent.
