| Objective:To explore the effect of nursing intervention based on Orem self-care theory on self-care ability,quality of life,self-efficacy and prognosis of diabetic patients in combination of medical and nursing institutions;It provides scientific basis for exploring the best nursing mode of diabetic patients in the combination of medical and nursing institutions.Methods:Using the convenient sampling method,60 patients with diabetes who were admitted to a municipal combined medical and nursing institution from September 2018 to November 2018 were selected as the control group and given routine nursing care.A total of 63 patients with diabetes who were admitted to the medical and nursing institution from January 2019 to March 2019 were selected as the intervention group.On the basis of Orem self-care theory,partial compensation measures and supportive education measures based on patients’ self-care ability were adopted.The self-care ability,quality of life,self-efficacy and the effect of disease recovery in the two groups were observed.SpSS 22.0 software was used for statistical analysis of the survey results.Results:1.Comparison of self-care ability of diabetic patients in the two groups:after three months of nursing intervention based on Orem self-care theory,the self-care ability scores of the intervention group and the control group were(124.38±21.55)and(109.90±20.98),statistically significant difference(P<0.01)2.Comparison of quality of life between the two groups of diabetic patients:after three months of nursing intervention,the quality of life scores of the intervention group and the control group were(99.76±19.49)and(109.80±21.90),statistically significant difference(P<0.01).3.Comparison of self-efficacy between the two groups of diabetic patients:after three months of nursing intervention,the average score of self-efficacy between the intervention group and the control group was(7.19±0.52)and(6.63±0.71),statistically significant difference(P<0.01).4.The blood sugar,glycosylated hemoglobin,body mass index(BMI)index comparison:three months after nursing intervention,intervention group and control group in fasting plasma glucose values respectively(6.81± 1.16)and(7.34±1.51),postprandial 2 h blood glucose values respectively(8.31 ± 1.27)and(9.34 ± 2.20),glycosylated hemoglobin values respectively(7.12±0.81)and(8.49±1.69),There were statistically significant differences among the three groups(P<0.05).;BMI was(25.03±2.11)and(24.31 ±2.12),respectively,with no statistically significant difference(p>0.05).5.Comparison of self-care ability before and after self-care intervention in the intervention group:the self-care ability before and after self-care education intervention in the partial compensation group was(90.14± 14.33)and(110.97± 18.07),respectively.paired t test was used,and the results showed that p=0.013,with statistically significant difference(p<0.05).The self-care ability of the support education group before and after intervention was(126.63±8.54)and(142.26±9.35),respectively.paired t test was used,and the results showed that p=0.006,with statistically significant difference(p<0.01).6.pearson correlation analysis showed that the self-care ability of diabetic patients was negatively correlated with the quality of life score(r=-0.455,p<0.01),the self-care ability was positively correlated with the sense of self-efficacy(r=0.691,p<0.01),and the correlation coefficient between the quality of life score and the sense of self-efficacy was r=-0.330,p<0.01.(note:quality of life score is negatively correlated with quality of life)Conclusion:1.Orem self-care theory is applied to the nursing intervention of diabetic patients in the combined medical and nursing institutions,which improves the patients’ self-nursing ability,life quality and self-efficacy,and improves the recovery of the disease,which has important clinical significance.2.The nursing model based on Orem self-care theory is based on patients’ self-care ability.It is an effective way to improve patients’ quality of life,and it is worth promoting and applying in the nursing of chronic diseases in medical and nursing institutions. |
