| Objective:To explore the distribution characteristics of TCM syndrome of premature beat and the correlation between the combined disease and TCM syndrome of premature beat,so as to provide reference for the clinical diagnosis and treatment of TCM of premature beat.Methods:Using the method of clinical epidemiological investigation,the clinical information of1849 patients with premature beats who met the inclusion criteria from three hospitals in Henan Hospital of traditional Chinese Medicine(the second affiliated Hospital of Henan University of traditional Chinese Medicine),the first affiliated Hospital of Henan University of traditional Chinese Medicine and the affiliated Hospital of Henan Institute of traditional Chinese Medicine from May 2010 to January 2020 were collected and the data were recorded into Epidat3.1 database.Multivariate statistical analysis was carried out by using IBM SPSS 19.0 software,and combined with TCM theoretical knowledge and expert experience,the main points of diagnosis of premature pulsation syndrome were established to explore the correlation between complicated diseases and TCM syndromes.Results:1 Among the cases included in this study,the most common complications of premature beating were coronary heart disease,essential hypertension,type 2 diabetes,hyperlipidemia and heart failure.2 The top 10 symptoms with the highest frequency of premature beating were palpitation,shortness of breath,fatigue,chest tightness,insomnia,fatigue,dreaminess,dizziness,irritability,dry mouth and so on.The tongue with the most frequent occurrence is dark red tongue,red tongue and red tongue tip.The tongue coating with the most frequent occurrence was thin,white,yellow and greasy.The pulse is mainly composed of sunken veins and veinlets.3 Using cluster analysis combined with factor analysis,1849 patients were divided into6 categories.Combined with professional knowledge,the names of various syndromes and their main symptoms were obtained.The first category is phlegm-fire disturbing heart syndrome,a total of 361 cases,the main four diagnostic information: chest tightness,shortness of breath,fatigue,dizziness,epigastric ruffian,receptive,forgetful,dry mouth and bitter mouth,insomnia,red tongue,yellow and greasy fur,slippery pulse,etc.The second category is yin deficiency fire syndrome,a total of 269 cases,the main four diagnostic information: palpitations,shortness of breath,fatigue,fatigue,dizziness,dizziness,irritability,forgetfulness,dry mouth,thirst and drinking,night sweats,insomnia,dreaminess,red tongue,sharp red,yellow fur,thin pulse,etc.The third category is the syndrome of deficiency of both qi and yin and blood stasis and heat,a total of 663 cases,the main four diagnostic information: palpitation,chest pain,shortness of breath,chest tightness,mental fatigue,fatigue,addiction,irritability,irritability,forgetfulness,dry mouth,insomnia,dreaminess,dark red tongue,thin or white or yellow coating,thin pulse,etc.The fourth category is heart blood stasis syndrome,a total of 278 cases,the main four diagnostic information: palpitations,chest pain,chest tightness,shortness of breath,fatigue,dizziness,forgetfulness,irritability,limb numbness,dreaminess,lip cyanosis,dark or dark red tongue,white fur,thin pulse,etc.The fifth category is deficiency of both heart and spleen syndrome,a total of 191 cases,the main four diagnostic information: palpitation,chest tightness,shortness of breath,mental fatigue,fatigue,dizziness,abdominal distension,forgetfulness,fear of cold,spontaneous sweating,dreamy,light tongue,white coating,pulse or heavy or thin or weak;The sixth category is heart-yang deficiency syndrome,a total of 87 cases,the main four diagnostic information: palpitation,chest tightness,chest pain,shortness of breath,dizziness,fatigue,dizziness,wheezing,forgetfulness,sore waist and knees,fear of cold,lukewarm hands and feet,lower limb edema,insomnia,light tongue,thin white fur,thin pulse and so on.4 The common syndromes of coronary heart disease complicated with extrasystole are qi-yin deficiency and blood stasis-heat syndrome,phlegm-fire disturbing heart syndrome and heart blood stasis syndrome.The common syndromes of hypertension complicated with extrasystole are phlegm-fire disturbing heart syndrome,qi-yin deficiency and blood stasis-heat syndrome.The common syndromes of type 2 diabetes mellitus complicated with extrasystole are qi-yin deficiency and blood stasis-heat syndrome,yin deficiency and exuberant fire syndrome.The common syndrome of hyperlipidemia complicated with extrasystole is the syndrome of phlegm-fire disturbing the heart.The common syndrome of heart failure complicated with extrasystole is the syndrome of heart blood stasis.5 The results of correspondence analysis showed that type 2 diabetes mellitus and functional extrasystole were closely related to the syndrome of exuberant fire due to yin deficiency and the syndrome of deficiency of qi and yin with blood stasis and heat.Essential hypertension and hyperlipidemia are closely related to the syndrome of deficiency of both heart and spleen and the syndrome of phlegm-fire disturbing the heart.Coronary heart disease and heart failure are closely related to heart blood stasis syndrome and heart yang deficiency syndrome.Conclusion:1 The syndrome of deficiency of qi and yin and blood stasis and heat is the most common syndrome of premature pulsation in traditional Chinese medicine,followed by phlegm and fire disturbing heart syndrome,heart blood stasis syndrome,yin deficiency and fire exuberant syndrome,heart and spleen deficiency syndrome,heart yang deficiency syndrome.2 Type 2 diabetes mellitus and functional extrasystole were closely related to the syndrome of exuberant fire due to yin deficiency and the syndrome of deficiency of qi and yin with blood stasis and heat.Essential hypertension and hyperlipidemia are closely related to the syndrome of deficiency of both heart and spleen and the syndrome of phlegm-fire disturbing the heart.Coronary heart disease and heart failure are closely related to heart blood stasis syndrome and heart yang deficiency syndrome. |
