Objective:Investigate the current status of core competence of ICU Clinical Nurse Specialist;To grasp cognition,attitudes and expectations among ICU medical staff,Master of Nursing Specialist’s on the joint training of ICU Clinical Nurse Specialist and Master of Nursing Specialist;To construct a training program for the integration of ICU Clinical Nurse Specialist and Master of Nursing Specialist.Methods:1.To understand and analyze the core competence level of ICU Clinical Nurse Specialist,386 nurses in which have been trained in the ICU clinical nurse specialist site of Jiangsu Province,are investigated by the convenience sampling method and surveyed by a core competency level questionnaire which developed by Zhong Li-ping.With the result of investigation analysis,the core competency framework for critical care,as the joint training goals has been developed.2.Based on the previous survey and literature review,a questionnaire is self-designed for identifying relational stakeholder’s perspective,attitude,expectations towards the joint training program between ICU clinical nurse specialist and master of nursing specialist.A survey is conducted among 95 ICU specialist nurses,40 ICU doctors,55 MNS to understand different stakeholders’ knowledge,attitude and needs,approach elements of the framework on the curriculum design,teacher requirements,and teaching and examination and evaluation methods of the joint training program.3.Two rounds Delphi method is used to develop and confirm the joint training program of ICU clinical nurse specialist and Master of Nursing Specialist.Integrated the results of previous study,compiled Delphi questionnaires based on the current ICU clinical nurse specialist training program and nursing master’s degree education program.Based on the results of the Delphi,combing with the actual critical care practice,determine the final joint training program.Result:1.According to the survey of the core competence and professional status on ICU Clinical Nurse Specialist in Jiangsu Province explores that the critical care nurses currently assume multiple roles in a care setting such as critical care nurse,nursing educationalist and manager.Meanwhile,the development of Acute Care Nursing Practice,Educational skill,Nursing Research skill,Management/Leadership skill,Consultation/Advisory skill is uneven,critical care practice skill is best,research skill is weakest,and intensive care related knowledge is insufficient.2.To investigate the ICU medical staff and nursing of master degree’s knowledge,expectations and needs on joint training,and the result explore that little knowledge of ICU Clinical Nurse Specialist training and MNS education among different stakeholder groups;the strength of ICU Clinical Nurse Specialist training is cultivation of ICU nursing specialist skills,the superiority of MNS education is that systematization of education and focus on comprehensive ability;the advantage of joint training is that it can improve critical care nursing level and achieve integration with education of acute care advanced practice nurse;improve intensive care management skill and research skill and so on;the joint training should break through obstacles such as training program,enrollment and admission qualification,curriculum assignment,criteria for the evaluation and assessment of CNS master’s program,and degree/certification criteria.3.The effective response rate of the two rounds of consultation were 90% and 95%;the individual expert authority coefficients were 0.73~1.0,the group expert authority coefficients were0.85;In the first round consultation,the coefficient of variation of program objective,curriculum assignment,clinical practice curricula,and criteria for the evaluation and assessment of CNS master’s program were 0.000~0.214、0.045~0.195、0.034~0.569、0.072~0.181,In the second round consultation,the coefficient of variation of four primary elements were 0.056~0.180、0.046~0.180、0.033~0.195、0.076~0.186;Two rounds coordination coefficient of first-grade indexes、second-grade indexes of program objective were 0.205~0.233、0.154~0.200(P<0.001),coordination coefficient of first-grade indexes、second-grade indexes of curriculum assignment were 0.286~0.292、0.209~0.248(P<0.001),coordination coefficient of clinical practice curricula,and criteria for the evaluation and assessment of CNS master’s program were0.279~0.352(P<0.001)、0.108~0.266(P<0.001);Construction of the joint training program for ICU Clinical Nurse Specialist nurses and Master of Nursing Specialist: it consists of program objective,curriculum assignment,clinical practice curricula,and criteria for the evaluation and assessment of CNS master’s program.The program objective including 8 first-grade indexes and 40second-grade indexes,the first-grade indexes including professional values,critical care basic medicine knowledge,critical care specialist practical skill,and emergency response ability and so on.The curriculum assignment is divided into 4 modules,including intensive care specialty courses,master of nursing public degree courses,master of nursing basic courses,master of nursing professional development courses,including 23 courses.The clinical practice program includes 11 items of intensive care practical goal,14 items of practical content,3 items of clinical arrangements,and 6 items of clinical teacher qualifications.The criteria for the evaluation and assessment of CNS master’s program include nursing master project: intensive care clinical topics,3 items of the evaluation and assessment methods,and 5 items of clinical practice examination methods.Conclusion:1.The core competence of ICU Clinical Nurse Specialist generally is in the middle upper ranks,with the highest score in Acute Care Nursing Practice,and the lowest is the dimension of Nursing Research ability,followed by Educational ability,Management/Leadership ability,and Consultation/Advisory ability.2.The project reveals the needs of the joint training program for ICU Clinical Nurse Specialist nurses and Master of Nursing Specialist from three stakeholder groups of ICU nurses,ICU doctors,and Master of Nursing Specialist.It formed a preliminary program for jointing cultivation,and applied Delphi method to construct the joint training program which including program objective,curriculum assignment,clinical practice curricula,and criteria for the evaluation and assessment of CNS master’s program,highlighting the specialization characteristics of intensive care,is scientific and reliable.
