ObjectiveTo explore the clinical efficacy of Buyang Huanwu Decoction combined with transverse tibial bone removal in the treatment of diabetic foot with Qi deficiency and blood stasis,and to provide a new idea for the treatment of diabetic foot with Qi deficiency and blood stasis.MethodsThe 80 patients with qi deficiency and blood stasis type diabetic foot(DF)who met the diagnostic criteria of the subject research and included in the criteria were divided into 2 groups using a random number table method,with 40 cases in each group.All patients in the two groups were subjected to detailed preoperative examinations.Consult the medical history in detail,check the body carefully,and record the results of VAS score,lower extremity vascular color Doppler ultrasound,and lower extremity CTA in detail.Actively reduce blood sugar,fight infection,and treat basic diseases.After blood sugar and infection are well controlled,transversal tibia bone removal is performed.And on the 5th day after surgery,bone transportation was carried out in accordance with the diagnosis and treatment standards.The control group continued hypoglycemic and anti-infective treatment after operation,and changed the dressing of the ulcer wound regularly.The test group was given Buyang Huanwu Decoction on the basis of the control group,and started taking it on the first day after the operation for 6 consecutive weeks.The VAS scores,the internal diameter of the posterior tibial artery,and the average blood flow velocity of the posterior tibial artery were recorded in the two groups at 6 weeks and 12 weeks after the operation,and the healing of the ulcer wound and the improvement of clinical symptoms were performed on the 12 th week after the operation.Evaluate the treatment effect according to the evaluation standard of Chinese medicine effect.Results1.Clinical Observation of Buyang Huanwu Decoction Combined with Tibia Transverse Bone Removal in Treating Diabetic Foot of Qi Deficiency and Blood Stasis Syndrome2.After 12 weeks of treatment,VAS score,posterior tibial artery inner diameter,and posterior tibial artery average blood flow velocity were compared between the two groups.VAS score: Compared within the group,the two groups were significantly lower than before the operation at 6 and 12 weeks after surgery(P<0.05).It can be seen from the comparison between the groups that the test group decreased more significantly than the control group(P<0.05).The inner diameter of posterior tibial artery: The two groups were significantly improved at 6and 12 weeks after surgery(P<0.05);comparison between the groups showed that the experimental group was better than the control group(P<0.05).The average blood flow velocity of the posterior tibial artery: both groups were significantly improved at 6 and 12 weeks after surgery(P<0.05);the test group was significantly improved compared with the control group(P<0.05).3.The effectiveness of traditional Chinese medicine was evaluated for the two groups of patients.The wound healing rate of the control group was 23%,the treatment effective rate was76%,the wound healing rate of the experimental group was 46%,and the treatment effective rate was 89%.<0.05).ConclusionsBuyang Huanwu Decoction combined with tibial lateral bone transport can effectively improve lower limb vascular function,promote lower limb blood circulation reconstruction,improve lower limb ischemia,relieve pain symptoms,promote ulcer wound healing and accelerate the clinical effect.It can effectively shorten the recovery time,thereby improving the quality of life and reducing the risk of amputation.It is worthy of promotion.
