| The 19 th CPC National Congress raised "healthy China" to a strategic height and set the goal of "strengthening the community-level health care service system and building a team of general practitioners,so as to provide people with all-round and full-cycle health services".The promotion and implementation of family physician contract service policy plays an important role in optimizing the allocation of medical resources,effectively controlling medical expenses and improving the level of medical services.Shanghai was the first city to pilot family physician contract services in 2011,and by the end of 2019,family physician contract services have been promoted nationwide.The implementation of the signing service policy for family physician has brought a lot of positive effects,but on the other hand,it has exposed some problems,such as low utilization rate of services,large gap of general practitioners,difficulty in fulfilling the contract,unreasonable allocation of primary medical resources,and low public satisfaction.On the basis of sorting out relevant domestic and foreign literature,this paper selected G district of fuzhou city as the research object,and used literature research method,questionnaire survey method and interview method,put forward by Horn-Meter model as the theoretical framework of policy implementation,to G district the family physician service contract to policy implementation present situation analysis,and summed up its is signed by the family physician service some problems existing in the policy execution.First of all,the effectiveness of policy implementation has failed to achieve the desired objectives.Second,the policy implementation mode cannot fully meet the target group needs.Third,there is selective implementation in the process of policy implementation.Fourth,the participation of target groups needs to be improved.Fifth,the low job satisfaction and career achievement of policy implementers.The main reason is that the setting of policy objectives and implementation standards is not perfect、insufficient allocation of policy resources 、 institutional constraints on policy implementation 、 poor communication and feedback mechanisms among policy implementers 、 objective conditions in the systemenvironment limit policy implementation.In view of this,the Horn-Meter model is used to put forward the optimization policy from the aspects of perfecting the policy objectives and setting standards,optimizing policy allocation,enhancing the executive power of policy implementing agencies,building a good system environment,perfecting the value orientation of policy implementers. |
