Objective:We analyzed the endoscopic manifestations,pathological features and cancerous characteristics of colorectal polyps to investigate the clinical and pathological features of colorectal polyps in different age groups.Methods:From January 2018 to October 2020,a total of 2,574 patients were diagnosed with colon polyps in the digestive endoscopy center of TISCO General Hospital,all of the patients who found colon polyps have undergone pathology.And we divided them into young group(18-39 years old),middle age group(40-59 years old)and old group(over60 years old),the gender,polyp distribution,number,pathological type and cancerous characteristics were compared.Results:(1)Gender and Age:Among the 2574 patients with colorectal polyps,there were1417 males and 1157 females,the male to female ratio was 1.22:1.The prevalence of polyps was significantly higher in males than in females.According to age,they were divided into young group,middle age group and old group.The ratio of male to female was 1.24:1 for the young group,1.32:1 for the middle-aged group and 1.12:1 for the elderly group,there was no statistically significant difference in the sex composition among the three age groups(P>0.05).Of the 2,574 patients enrolled,2,548 were pathologically diagnosed with colorectal polyps and 26 were pathologically diagnosed with colon cancer.(2)Number of polyps:Among the 2574 patients with colorectal polyps,there were1012(39.7%)patients with single polyps and 1536(60.3%)patients with multiple polyps.The proportion of multiple polyps was 38.16% in the young group,57.59% in the middle age group,and 65.03% in the old group.With the increase of age,the proportion of multiple polyps gradually increased,the difference was statistically significant(P<0.05).(3)Polyp position:Colorectal polyps in the three age groups were mainly distributed in the left colon.With the increase of age,the proportion of polyps in the left colon decreased,while the proportion in the whole colon increased,and the difference was statistically significant(P<0.05).(4)Pathological feature:A total of 5320 polyps were detected in 2548 patients with colorectal polyps,all of which were mainly inflammatory polyps.With the increase of age,the proportion of adenomatous polyps increased significantly,and the difference was statistically significant(P<0.05).(5)Hyperplasia:468 of the 2,574 patients had intraepithelial neoplasia.With the increase of age,the rate of high-grade intraepithelial neoplasia increased gradually,but age was not associated with the degree of dysplasia.(6)Cancerous traits:The cancer mainly occurred in the left colon,accounting for73.08%(19/26).The cancer rate of left colon was significantly higher than that of right colon.Women are more likely to develop cancer than men.The difference between the two groups was not statistically significant(P>0.05).The cancer rate of the elderly group was significantly higher than that of the young and middle-aged group,the difference was statistically significant(=9.013,P=0.011).Conclusion:(1)Age were correlated with the location,number and pathologic types of colonic polyps.(2)Cancer has nothing to do with location and sex,but is closely related to age.Middle-aged and elderly patients should pay attention to the observation of the whole colon.(3)Early detection and removal of lesions,early detection and resection of the lesions is of great significance for early detection and blocking of the development of colon cancer.
