Objective:To observe the clinical efficacy and safety of Buyuan Juanxiao Decoction in the treatment of chronic obstructive pulmonary disease(COPD)in the stable period(pectoral Qi deficiency and failure, syndrome of phlegm and blood stasis obstructing lung),as well as probe into its conceivable mechanism of action,so as to provide more options for the clinical treatment of COPD at stable stage. Methods:From January 2020 to January 2021,60 patients with COPD in stable phase(pectoral Qi deficiencyand failure, phlegm and syndrome of phlegm and blood stasis obstructing lung)who met the inclusion criteria and were screened by exclusion criteria in the outpatient department of Respiratory and Critical Care Medicine of the Affiliated Hospital of Jiangxi University of Chinese Medicine, including 30 patients in the treatment group and 30 patients in the control group. The control group was treated with conventional western medicine, while the treatment group was additionalized with Buyuan Juanxiao Decoction, with a course of 3months.To observe the effects of Buyuan Juanxiao Decoction on the TCM syndrome total score,TCM symptom score of single item, pulmonary function(FEV1/FVC,FEV1%pred),CCQ score,CAT score, exercise endurance assessment(6MWD)and other indexes of patients with COPD at stable stage(pectoral Qi deficiency and failure, syndrome of phlegm and blood stasis obstructing lung). The results were analyzed using SPSS24.0 statistical software to evaluate the difference in efficacy between the two groups.Results:1.The total TCM syndrome score,CAT score and 6MWD were distinctly ameliorate in both groups after treatment(P<0.05),whereas the ameliorate level in the treatment group was superior to that in the control group(P<0.05).2.Comparison of individual TCM symptom scores:(1)Comparison within the group: The scores of individual symptoms in the treatment group were all cut down(P<0.05);after treatment in the control group, only cough, wheezing, shortness of breath, extreme movements and cyanosis of the lips were cut down(P<0.05),but there were no distinctly alter in expectoration, loss of appetite, and fatigue(P>0.05).(2)Comparison between groups: The reduction in scores of the treatment group in terms of cough, expectoration, decreased appetite, fatigue, and cyanosis of the lips was greater than that of the control group(P<0.05).3.Curative effect analysis: the total effective rate of the treatment group was 86.67%,while control group was 73.33%.The curative effect of the treatment group was superior than that of the control group(P<0.05).4.During the observation period of this clinical study,2 patients in the treatment group developed mild diarrhea, which returned to normal after treatment with rhubarb, without clinical significance; There were no abnormalities in three routine procedures, liver and kidney function and ECG in the two groups.Conclusion:1.Buyuan-Juanxiao Decoction Contribute to ameliorate the clinical efficacy and symptoms of patients with stable phase of COPD(pectoral Qi deficiency and failure, phlegm and blood stasis obstructing lung),especially in cough, sputum, reduced food consumption, fatigue, and cyanosis of lips, which is better than conventional Western medicine treatment alone.2.Buyuan Juanxiao Decoction Contribute to ameliorate the lung function(FEV1%Pred,FEV1/FVC)of patients with stable phase of COPD(pectoral Qi deficiency and failure, phlegm and blood stasis obstructing lung).3.Buyuan Juanxiao Decoction Contribute to ameliorate the exercise endurance of patients with stable phase of COPD(pectoral Qi deficiency and failure, phlegm and blood stasis obstructing lung).4.Buyuan Juanxiao Decoction can Contribute to cut down CCQ scores,CAT scores,mMRC scores in stable phase of COPD(pectoral Qi deficiency and failure, phlegm and blood stasis obstructing lung).5.Buyuan Juanxiao Decoction is safe and effective therapy of stable phase of COPD(pectoral Qi deficiency and failure, phlegm and blood stasis obstructing lung).
