Objective: To observe and compare the curative effect of flexion and rotation lifting manipulation and conventional traction in the treatment of cervical spondylotic radiculopathy,and compare the improvement degree of cervical spine arch-string distance between the two treatment schemes,so as to provide an optimized flexion rotation lifting manipulation treatment scheme for cervical spondylotic radiculopathy.Methods: According to the established inclusion criteria and exclusion criteria,80 cases of CSR were included.The time of treatment was from December 2018 to October 2020,and the place of treatment was orthopedic clinic or spinal orthopedic ward II of our hospital.The patients were randomly divided into two groups: the treatment group(n=40)and the control group(n=40).The clinical efficacy,symptoms,signs,functions of cervical spine and fingers,cervical curvature of the two groups were evaluated by visual analogue scale(VAS),CSR20 scale and cervical spine arch-string distance by lateral radiographs of cervical spine.All data were analyzed by SPSS 26.0 statistical software.Result:1.Before treatment,the data of the two groups were balanced,and the difference was not statistically significant(P>0.05).It was mainly reflected in the baseline data,CSR20 score,cervical spine arch-string distance and so on.2.After the course of treatment,the therapeutic effect was graded:the total effective rate was 92.50% and the markedly effective rate was87.50% in the flexion rotation lifting manipulation group,in which 24 cases were cured and 11 cases were markedly effective;the total effective rate and markedly effective rate were 80.00% and 45.00% in the conventional traction group,and the number of cured and markedly effective cases were 8 and 10 cases respectively.The difference was statistically significant(P<0.05).3.VAS score comparison: at the end of the course of treatment,the VAS score in the flexion rotation lifting manipulation group decreased more significantly,and the difference was statistically significant(P<0.001).Before treatment,the VAS score of the treatment group was 5.63±0.84)and that of the control group was(5.65±0.89);after treatment,the VAS score decreased to(1.20±1.45)and(2.43±1.72)respectively;4.CSR20 total score and each score(before treatment and after two weeks of treatment)comparison: CSR20 total score of flexion rotation lifting manipulation group before treatment and after two weeks of treatment were(9.38±3.74),(16.70±4.15);before treatment and after two weeks of treatment,the total score of CSR20 in conventional traction group was(8.95±4.22),(14.13±3.53).Before treatment,the score of CSR20 in flexion rotaion lifting manipulation group was(4.38±1.21),(-1.25 ± 0.63),(4.45 ± 1.30),(1.53 ± 0.84);the scores of routine traction group were(4.20±1.38),(-1.40±0.67),(4.75±1.51),(1.40±0.84).After 2 weeks of treatment,the scores of the flexion rotation lifting manipulation group were(7.35±1.56),(-0.33±0.47),(6.93±0.97),(2.50±0.75);the scores of the conventional traction group were(6.68±1.26),(-0.68±0.52),(6.23±1.04),(1.88±0.93).The differences between the above groups were statistically significant(P<0.05).5.At the end of the treatment,the cervical spine arch-string distance of the two groups was increased to a certain extent,and the difference was statistically significant(P < 0.05).In the flexion rotation lifting manipulation group,the cervical spine arch-string distance was(5.38 ± 4.75mm)before treatment,after the course of treatment the distance was(6.41±4.65mm)and the difference of arcuate distance was(1.04±0.24mm)after the course of treatment;the cervical spine arch-string distance was(5.73±4.61mm)before the treatment and(6.26±4.51mm)and(0.51±0.14mm)after the course of treatment in the conventional traction group.There was significant difference in the difference of cervical arch chord distance after treatment(P<0.001).Conclusion:1.Flexion rotation lifting manipulation and conventional traction are effective,safe and reliable.Flexion rotation lifting manipulation is superior to conventional traction in symptoms,signs,cervical and hand functions,cervical curvature changes,etc.2.Flexion rotation lifting manipulation treatment has the advantages of high effective rate,painless,high patient satisfaction,and significantly improve the quality of life;The transformation of the research results can bring huge benefits to patients and reduce medical expenses,which is worthy of clinical promotion and application.
