Study On The Effect Of Two Levels Of PH Electrolyzed-oxidizing Water On Oral Care Before Gastrointestinal Surgery Patients

Objective:Methods:Clinical randomized controlled study was used to select patients who underwent gastrointestinal surgery in a tertiary general hospital in Hebei Province from December 2019 to October 2020.The random number table method was divided into experimental group A,experimental group B and control group.Group A was given EOW of pH 2.0 ~ 3.0,group B was given EOW of pH 6.0 ~ 7.0,and the control group was given saline.Each gargling amount was about 30 m L,which was carried out in the morning,30 min after three meals,and before bed,respectively.The intervention lasted for 3 days starting 3 days before the operation.Before the first gargle and 5 minutes after the last gargle,take the patient’s throat swab specimens for microbiological culture(colony count and fungal culture),measure the oral pH,measure the plaque index(PLI),and observe the patient whether there are adverse reactions.SPSS 23.0 was used to analyze the data.Results:To explore the effect of electrolyzed oxidizing water(EOW)with pH2.0~3.0 and pH 6.0~7.0 on preoperative oral care for patients undergoing gastrointestinal surgery,so as to provide reference basis for the follow-up application of EOW in clinical oral care.1.A total of 113 patients were included in the study,including 37 cases in the experimental group A,38 cases in the experimental group B and 38 cases in the control group.There was no significant difference in the general demographic data and clinical data among the three groups(P>0.05).And the baseline data was balanced and comparable.2.Oral colony count culture results3.Oral fungal culture resultsIn group A,9 cases were positive and 28 cases were negative before oral care.After oral care,8 cases were negative in the 9 cases,1 case was still positive.The 28 cases were negative after oral care.There were 8positive cases and 30 negative cases in group B before oral care,and all the results were negative after oral care.In the control group,5 cases were positive and 33 cases were negative before oral care.After oral care,4 cases were still positive,1 case turned negative,and the 33 cases were all negative after oral care.Compared with the paired samples before and after oral care in the group,the positive rate of fungi in group A and group B decreased after oral care,and the difference was statistically significant(P<0.05).While there was no significant difference in the control group(P>0.05).Further comparison between group A and group B showed no statistically significant difference(P > 0.05).4.Oral pH test resultsThe colony count results were analyzed after logarithmic transformation(X’=lg X).Before and after oral care,oral colony count of the experimental group A,the experimental group B and control group decreased,the difference was statistically significant(P<0.05).There was no significant difference in the colony count results before oral care among the three groups(P>0.05).The difference of colony count between the three groups before and after oral care was compared between the groups.The difference of colony count in group A and group B was higher than that in the control group(P<0.05).There was no significant difference between group A and group B(P>0.05).Compared with the paired samples before and after oral care.The oral pH of experimental group A decreased,and the difference was statistically significant.There was no significant difference between experimental group B and the control group(P>0.05).There was no significant difference in the5.PLI measurement results6.No adverse reactions,such as irritation of oral mucosa,numbness of tongue and change of taste,were observed in the three groups after using mouthwash.Conclusion:EOW of pH 2.0~3.0 and EOW of pH 6.0~7.0 as mouthwash for preoperative oral care of patients undergoing gastrointestinal surgery can improve oral hygiene better than normal saline,which is commonly used in clinic.It is proved that EOW gargle has good germicidal effect in addition to mechanical clearance.There was no different effect in oral mucosal disinfection(colony count,fungal culture)between EOW of pH 6.0~7.0 and EOW of pH 2.0~3.0.EOW of pH 6.0-7.0 was more effective in removing dental plaque.Moreover,the effect of pH 6.0~7.0 EOW on oral pH was consistent with that of normal saline,which was closer to the normal oral microenvironment.Therefore,EOW of pH 6.0~7.0 has better oral care effect than EOW of pH 2.0~3.0.No adverse reactions were found in the use of EOW during the study period.Comparison of PLI between before and after oral care in group A,group B and control group showed statistically significant differences(P<0.05).There was no statistically significant difference in the PLI of the three groups before oral care(P>0.05).The difference in PLI of the three groups before and after oral care was compared between the groups.The PLI of group B was higher than that of group A and the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference between group A and the control group(P>0.05).pH before oral care among the three groups(P>0.05).The pH difference before and after oral care among the three groups was compared.The pH difference in group A was higher than that in group B and the control group.The difference was statistically significant(P<0.05).There was no significant difference between group B and the control group(P>0.05).EOW with pH 6.0~7.0 for preoperative oral care of patients undergoing gastrointestinal surgery can safely and great improve the oral hygiene of patients.And it is expected to be further studied in clinic.
