Analysis On Equity And Efficiency Of Medical Resource Allocation In HCM Hospitals In Tianjin From 2011 To 2018

ObjectiveIn order to clarify the situation and level of medical resources allocation of Chinese medicine hospital in Tianjin,this paper,starting from the current situation of medical resources in Chinese medicine hospital in Tianjin from 2011 to 2018,analyzes the fairness and efficiency of medical resource allocation and predicts the medical resources from 2019 to 2028,thus revealing the characteristics and outstanding problems of medical resources in Chinese medicine hospital in Tianjin,and puts forward targeted suggestions and countermeasures to provide scientific basis for making development plans and coordinating the allocation of various types of medical resources.MethodsThrough descriptive analysis method,the medical resources and allocation of Chinese medicine hospital in Tianjin were understood,the fairness and regional allocation of medical resources were analyzed by population,geographical allocation Gini coefficient,Lorenz curve and Ter index,the effectiveness and allocation efficiency of medical resources were analyzed by packet analysis,and the future development of medical resources was predicted by gray model method.Results1、Tianjin Chinese medicine hospital medical resources status quo:(1)from 2011 to 2018,Tianjin Chinese medicine hospital medical resources are showing an overall growth trend.All types of health technicians showed an increasing trend,with an average annual growth rate of6.05% for practicing(assistant)physicians,7.30% for registered nurses,3.33% for pharmacists and 3.92% for technicians.(2)Among the health technicians,the number of practicing assistant physicians is large,accounting for an average of 45.86% of the total number,higher than the25 to 35% stipulated in the Hospital Staffing Ratio and Staffing,and the average medical care ratio is 1:0.855,which is lower than the target of 1:1.25 of the "13th Five-Year Plan" for Health and Health Care.(3)The overall growth trend of the total number of general consultations,outpatient visits and emergency visits indicates that the capacity of service diagnosis and treatment has improved.(4)Both the number of admissions and discharges showed an increasing trend,and the utilization rate of hospital beds and average hospitalization days showed a general downward trend,indicating that medical capacity and consultation procedures were optimized.2、Tianjin TCM Hospital medical resources allocation equity analysis:(1)according to the population allocation of various medical resources Gini coefficient is relatively fair,according to the geographical allocation of various medical resources Gini coefficient gap,both show a year-on-year growth trend.(2)The total difference of the Theil Index shows that the medical resources are fair,but show an increasing trend year by year.The difference of institutional Ter index mainly comes from the difference between regions,and the difference of bed,practicing(assistant)physician,registered nurse and technician Theil index mainly comes from regional difference.3、Tianjin TCM Hospital medical resource allocation efficiency analysis:(1)2011,2012,2014 to achieve the effective state of the DEA;(2)The decline in scale gains in 2013,2015,2016,2017 and 2018 demonstrates a decrease in output as the scale of medical resource inputs increases.4、Tianjin TCM Hospital medical resources allocation trend forecast: It is expected that by2028,Tianjin TCM Hospital medical institutions,number of beds,the number of practicing(assistant)physicians,the number of registered nurses,pharmacists,technicians are 119,18871,10 336,9044,1278 and 749.The average annual growth rates of were 7.14%,6.60%,6.80%,6.87%,2.78% and 3.44%,respectively.ConclusionFrom the current situation of allocation,all medical resources of Tianjin TCM Hospital have shown the growth of the past year,but in the allocation of human resources of Chinese medicine,there are problems such as unreasonable structure of health and technical personnel,inverted medical care ratio,from the point of view of allocation equity,the Gini coefficient by population allocation is better than the Gini coefficient according to geographical allocation,indicating that there is a problem of gathering in densely populated areas,dispersing in sparsely populated areas,and the allocation of resources has a tendency to develop in an unfair direction;And the inefficient allocation of resources mainly stems from the lack of scale efficiency,resulting in decreasing scale income,from the resource forecast situation,Tianjin TCM Hospital medical resources have shown an increasing trend over the years,but the proportion of practicing(assistant)physicians is too high,the number is too large,the ratio of medical care is upside down,the number of pharmacists and technicians growth rate is small,accounting for a relatively low number of problems still exist.Therefore,this paper suggests that the relevant institutions improve the allocation of medical resources of Chinese medicine medical institutions in Tianjin by strengthening the construction of nursing team,adjusting the structure of health and technical personnel,paying attention to the allocation of medical resources in remote areas,using the Internet platform for online consultation,controlling the blind expansion of hospital scale,reasonably reducing the investment of medical resources,and exploring and exerting the characteristics of Chinese medicine.
