Research Background:With the increasing level of the people,the prevalence of gout has increased year by year.The early symptoms of gout are not obvious or can heal after the onset of symptoms,which is easy to be ignored,and the blood uric acid level of most gout patients has not been effectively controlled.As the course of the disease progresses,Gout can cause damage to joints,blood vessels,and kidneys.Among them,Gouty Nephropathy(GN)is a common complication in patients with gout.In addition to the blood uric acid level,other factors that cause gouty nephropathy may still exist,such as the course of the disease,age,smoking,drinking,blood pressure,blood sugar,and blood lipids.Studies have shown that Chinese medical physique plays an important role in the occurrence and development of diseases,providing clinical reference and treatment basis.Therefore,this study analyzes the related risk factors and physical types of GN in patients with gout,with a view to early intervention in patients with gout to prevent the occurrence of GN or delay the progress of GN.Research purposes:Observe the differences between the physique type,clinical data,and related laboratory indicators of patients with gouty nephropathy and those with simple gout,look for risk factors for kidney damage in patients with gout,and provide evidence for the prevention and treatment of gouty nephropathy.research method:A total of 164 gout patients who attended the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from December 2019 to December 2020 were collected.According to whether kidney damage occurred,they were divided into 101 cases in simple gout group and 63 cases in gout nephropathy.The researcher uses a unified questionnaire to collect the physical type,clinical data and laboratory indicators of the research object.Use Excel to establish a database,and use SPSS 21.0 statistical software for data analysis.Analyze the data between the two groups according to the corresponding single factor analysis method.Multivariate binary Logistics regression analysis was used for statistically significant single factors,and P<0.05 was considered as the difference was statistically significant.Research result:1.Gouty nephropathy is determined to be blood stasis in 18 cases(28.1%),damp-heat quality in 11 cases(17.2%),phlegm-dampness in 5 cases(7.9%),Qi deficiency in 3 cases(4.7%),Yang Deficiency in 3 cases(4.7%),Qi stagnation in 3 cases(4.7%),Yin deficiency in 1 case(1.6%),idiosyncratic in 1 case(1.6%),and calm in 9 cases(14.1)%),the tendency is 20 cases(31.3%)with damp-heat constitution,18 cases(28.6%)with phlegm constitution,14 cases(21.9%)with blood stasis constitution,14 cases(21.9%)with yang deficiency constitution,qi depression There were 14 cases of constitution(21.9%),12 cases of qi deficiency(18.8%),8cases of yin deficiency(12.5%),3 cases of idiosyncratic constitution(6.3%),and 13 cases of basic peace(20.3)%).2.Comparing the physical types of patients with gout nephropathy and simple gout patients,the results show that patients with gout nephropathy have yang deficiency physique 14(21.9%)and tend to be yang deficiency physique 3(4.7%),qi deficiency physique 12(18.8%)And the constitution with tendency of Qi deficiency 3(4.7%),the constitution with blood stasis 14(21.9%)and the constitution with tendency of blood stasis 18(28.1%)were higher than those in the simple gout group,the difference was statistically significant(P<0.05).The number of patients in the GN group was 13(20.3%)and 9(14.1%)were basically peaceful and less than those in the gout group.The difference was statistically significant(P<0.05).However,there was no significant difference in Yin deficiency,phlegm-dampness,damp-heat,idiosyncratic,and Qi-stagnation(P>0.05).3.Univariate analysis results: There are statistically significant differences between patients with gouty nephropathy and patients with simple gout in age,course of disease,joint involvement,frequency of attacks,history of smoking,combined tophi,hypertension,diabetes,etc.(P<0.05).However,there were no statistically significant differences in gender,family history,drinking history,and coronary heart disease history(P>0.05).Comparison of laboratory indicators between the two groups showed statistically significant differences in fasting blood glucose,cholesterol,low-density lipoprotein cholesterol,red blood cells,hemoglobin,and urine pH(P<0.05).However,there was no significant difference in alanine aminotransferase,aspartate aminotransferase,triglycerides,high-density lipoprotein cholesterol,white blood cells,platelets,etc.(P>0.05).4.The statistically significant results of single factor analysis were subjected to multivariate binary logistic regression analysis.The results showed that combined tophi(OR=6.523,P=0.006),history of hypertension(OR=3.267,P=0.036),cholesterol(OR=2.342,P=0.018)and urine pH(OR=0.294,P=0.045)are risk factors for gouty nephropathy in patients with gout.Tophi,hypertension,and cholesterol are positively correlated with the risk of gouty nephropathy.Urine The pH value is negatively correlated.in conclusion:1.Gouty nephropathy is mainly composed of blood stasis,damp-heat,phlegm and dampness.Most patients have a combined constitution.Compared with patients with simple gout,they tend to be yang-deficiency,qi-deficiency,and blood stasis.2.Age,history of smoking,history of drinking,course of disease,joint involvement,frequency of attacks,tophi,high blood pressure,diabetes,fasting blood sugar,cholesterol,low-density lipoprotein cholesterol,urine pH,red blood cells,hemoglobin and the occurrence of gouty nephropathy related.3.Hypertension,tophi,high total cholesterol,and low urine pH are the risk factors for gouty nephropathy in patients with gout.
