ObjectiveData mining method was used to excavate the prescription rule,medication characteristics and core prescription of Professor Fu Bin in the treatment of gouty arthritis(GA),and network pharmacology technology was used to explore the mechanism of action of core prescription,in order to guide clinical practice.Methods1.Select the medical record data of GA patients who were in the outpatient clinic of Professor Fu Bin from the Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from January 2013 to December 2020.Screening,summarizing and sorting the collected medical record data,inputting it into Excel 2007 software,completing the establishment of the database.Use Excel 2007,IBM SPSS Modeler 18.0,IBM SPSS Statistics 26.0 software to carry out statistical analysis,and dig out Professor Fu Bin’s prescription rules,medication characteristics and core prescriptions for the treatment of GA.2.Network pharmacological analysis was conducted on Professor Fu Bin’s core prescription for the treatment of GA,and the effective ingredients and potential protein targets of the drugs in the core prescription were obtained by TCMSP,Batman-TCM and Uniprot database.Ga-related targets were screened from Gene Cards and OMIM databases.Venny was used to calculate common targets between the core prescription and the disease.The drug-chemical-target-disease network was constructed by Cytoscape 3.7.2.Through the STRING database,the protein interaction information network diagram was obtained.R language and Cytoscape 3.7.2 software were used to carry out topological analysis on the drug targets and screen out the core genes.Go function enrichment and KEGG signaling pathway analysis were performed on the common targets of core prescription and disease,so as to speculate the mechanism of action of core prescription on GA.Results1.Gender and age distribution: A total of 96 patients were included in this study,including 87 males and 9 females;The largest age group of patients at onset is 50 to 59 years old.2.Analysis results of drug frequency,efficacy,four qi and five flavors,and return to the channel: A total of 196 prescriptions were screened in this study,involving 115 drug flavors,with a total of 2864 times of drug use.The top 10 traditional Chinese medicines were soil poria cocos,licorice,Radix Sopranae,Chinese Medicine,Phellodendron phellodendron,bark of Qin Qin,honeysuckle vine,anemarrhexis anemarrhexis,angelica sinensis and salvia miltiorrhiza;The drug efficacy mainly involves heat-clearing drugs,defect-tonifying drugs,water-moistening drugs,blood-activating drugs and stasis removing drugs,and wind-dampness removing drugs.The proportion of the four qi is flat,cold,warm,cool and hot.The proportion of the five flavors was bitter,sweet,spicy,light,sour,astringent and salty.The top 5 meridians were liver,kidney,spleen,lung and stomach.3.Analysis results of drug association rules: among the association rules of the second drugs,the pairs of Ligestrium Ligestrium and Ligestrium moxa had the highest confidence degree,followed by Prunus oleifera,Dioscorea septum,Phellodendron chinense and Radix Radix Radix,etc.Among the correlation results of the three drugs,the drugs with strong correlation results included Radix Phelendra,Radix Radix Radix Radix Radix,Radix Licorice,Radix Anemarhizae,etc.4.Clustering analysis results of high-frequency drugs: Clustering analysis was performed on the top 35 Chinese herbal medicines in application frequency,and 6 drug combinations were obtained,among which the first category was Purea cocos,Radix Chrysalis Radix,Oral Chinese Medicine,Phelendron chinense and Anemarhizoma Rhizoma;The second category is licorice and gardenia;The third category is Qin Bark,Angelica sinensis,Coix seed,Atractylodes tractylodes,Tangerine peel,Scutellaria scutellariae,Deer’s grass,Qingfengteng,scorpion;The fourth category is honeysuckle vine,lily,tuckahoe,radish seed and Qiang fruit.The fifth category is Salvia miltiorrhiza,Polygonum cuspidatum,Rhubarb charcoal,Peilan orchid,Eucommia ulmoides,Fenugreu,Shanci mushroom,Rhubarb;The sixth group is Epimedium,Gentiana macrophylla,Rhizoma corydalis,Ligustrum Ligustrum and Mulberry branch.The core prescription is class I drugs.5.Network pharmacological study results: The main active components in the core prescription for GA treatment were quercetin,kaempferol,β-sitosterol and phytosterol;The core genes were IL6,VEGFA,IL1 B,PTGS2,Jun,CCl2,etc.GO functional enrichment analysis showed that the core prescription had molecular functions of regulating cytokine activity,cytokine receptor binding,growth factor receptor binding,etc.KEGG pathway analysis showed that IL-17,Toll-like receptor and other signaling pathways were the main pathways for the core prescription to play its role.Conclusion1.High frequency drugs commonly used by Professor Fu Binin in the treatment of GA are soil tachycocos,licorice,Radix Radix Sopranae,Chinese Dioscorea,Phelendron chinense,bark of Qin Qin,honeysup vine,anemarrhenae,angelica sinensis and salviae miltiorrhiza,etc.2.Professor Fu Bin’s common drugs for GA include Ligastia Ligastii and Ligastii moxa;Soil poria cocos,Achyranthes bidentata;Poria cocos,Phellodendron phellodendron,Radix Achyranthes bidentata;Soil Poria cocos,Radix Achyranthes bidentata,Anemarrhenae and so on.3.Professor Fu’s core prescription for the treatment of GA consists of 5 medicines,including soil porphyra cocos,Radix Sopranae Radix,Chinese Medicine,Phelendron chinense and Rhizoma Rhizoma Rhizoma.4.Based on network pharmacological methods,the core prescription for GA treatment is mainly through quercetin,kaamol,β-sitosterol,phytosterol and other potential active ingredients acting on IL6,VEGFA,IL1 B,PTGS2,CCl2 and other genes.In order to regulate the activity of cytokines,cytokine receptor binding,growth factor receptor binding and other biological functions,as well as IL-17 signaling pathway,Toll-like receptor signaling pathway and other pathways to achieve therapeutic effect.It is suggested that the mechanism of core prescription for GA may be the regulation of inflammatory response.
