Research On The Optimal Scheme For The Treatment Of Nerve Root Cervical Spondylosis Based On Orthogonal Design

Objective Analyze the law of selecting acupoints for the treatment of cervical spondylotic radiculopathy by exercise acupuncture;clinical trials are carried out on this basis,and the orthogonal design method is used to observe remote acupoint selection(Houxi,Zhongzhu)and local manipulation(resistance acupuncture,The clinical curative effect of different combinations of three factors and two levels in the treatment of cervical spondylosis of radiculopathy,screening of exercise acupuncture based on the above-mentioned levels of different factors,and the timing of acupuncture(<4 weeks,4-12 weeks)The optimized plan of cervical radiculopathy provides a basis for clinical treatment.Method 1 Through computer and manual search,collect China National Knowledge Infrastructure(CNKI),Wanfang Data Knowledge Service Platform(WANFANG DATA),VIP China Science and Technology Journal Database(VIP))All publicly published(as of April 15,2020)randomized controlled clinical research literature on exercise acupuncture treatment of cervical spondylotic radiculopathy in the three major databases,and establish an Excel database for exercise acupuncture treatment of cervical spondylotic radiculopathy,Analyze and summarize the law of acupoint selection.2 Using pre-designed remote acupoint selection(Houxi and Zhongzhu),local manipulation(resistance acupuncture,flattening and relieving method),acupuncture timing(<4 weeks,4-12 weeks),three factors and two levels According to the orthogonal design plan L8(27),40 patients with cervical spondylotic radiculopathy who meet the criteria for discharge were randomly divided into 8 groups,and the patients in each group were treated separately on the basis of the same conventional treatment.After treatment,record the pain visual analog scale(VAS)and NPQ neck pain scale(Northwick Park Neck Pain Questionnaire,referred to as NPQ% scale)of each group of patients;Tanaka Yasuhisa cervical spondylosis symptom scale 20 points method,and statistics analysis.Result 1 Literature research(1)A total of 26 randomized controlled clinical research literatures were included,involving 32 acupoints,including 30 acupoints in the fourteen meridians and 2acupoints outside the meridian.The total frequency of application was 59.The first two acupoints with the highest frequency of use were respectively Houxi(12 times)and Zhongzhu(9 times).(2)The selected acupoints involve a total of 13 meridians,including 12 serious meridians and 1 odd meridian(Governor meridian),of which the Hand Sun Small Intestine Meridian(13%),Hand Taiyin Lung Meridian(13%),Hand Shaoyang Sanjiao Meridian(10%),Hand Yangming Large Intestine Meridian(10%)is used more frequently.(3)The selection of acupoints is mainly based on specific acupoints,and the frequency of use is up to 36.Among them,the Shu acupoint is the most frequently used(31%),followed by the Eight-Mai Jiaohui acupoint(17%).2 Clinical trials(1)After two weeks of exercise acupuncture treatment of cervical spondylotic radiculopathy,the VAS score,NPQ% score and 20-point method score of each group decreased.Compared with before treatment,the difference was statistically significant(P<0.05).(2)From the results of the VAS score,the primary and secondary relationship of various factors in the whole treatment process is: acupuncture timing>distal acupoint selection>local manipulation.Acupuncture timing is the most significant factor affecting pain intensity,and it is less than 4 weeks.The clinical effect is the best.Acupuncture at the distal end of Houxi point,local resistance acupuncture method,acupuncture timing <4 weeks,the best effect on improving pain intensity.(3)From the NPQ% score results,the timing of acupuncture is the most significant factor affecting the NPQ% score,less than 4 weeks is better than 4-12 weeks.Resistance acupuncture has a greater impact in early treatment,and the impact of remote acupoint selection after one week of treatment is greater than that of local manipulation.(4)From the results of the 20-point method,the optimal combination that affects the score of the 20-point method is to select the Houxi point at the distal end,and the resistance acupuncture method is used locally,and the acupuncture timing is <4weeks.From an overall point of view,the local technique has the most significant impact on the score of the 20-point method,and the resistance acupuncture method is better than the flattening and reducing method.Conclusion1 Literature research Exercise acupuncture treatment of cervical spondylotic radiculopathy mostly adopts the principle of selecting acupoints along the meridian,focusing on specific acupoints,with Shu acupoints being the most,and the commonly used acupoints are Houxi and Zhongzhu.2 The optimal plan for the treatment of cervical spondylotic radiculopathy with exercise acupuncture is to select Houxi or Zhongzhu at the distal end on the basis of differentiation of symptoms and signs,and use resistance acupuncture locally,with acupuncture timing within 4 weeks.
