| Objective:By comparing the changes of primary and secondary syndrome scores of functional abdominal pain in children(stagnation of qi stagnation syndrome),this study observed and evaluated the clinical efficacy of massage manipulation based on invigorating the spleen and relieving stagnation in the treatment of functional abdominal pain in children(stagnation of qi stagnation).To explore the safety and effectiveness of massage manipulation for the treatment of functional abdominal pain in children,in order to provide reference for the external treatment of functional abdominal pain in children.Method:1.A single-center,randomized controlled clinical trial was used in this study.According to the standard of nano-exclusion,64 children were randomly divided into treatment group(n= 32)and control group(n = 32).The control group was treated with dried Hippophae rhamnoides emulsion,and the treatment group was treated with Yunpi Xiaozhu massage on the basis of treatment in the control group.2 weeks as a course of treatment,a total of 2courses of treatment.The observation records were made at the 2nd and 4th week of treatment.The curative effect evaluation takes the curative effect of the main disease as the main index,the sum of the main symptom score,the total score of TCM syndrome,the curative effect and the score of various symptoms as the secondary index,and the incidence of adverse reactions as the safety index.2.To study the mechanism of Hippophae rhamnoides in the treatment of functional abdominal pain in children based on network pharmacology.The effective components of Hippophae rhamnoides were retrieved by TCMSP database and imported into Drugbank database to obtain the targets of effective components.The related genes of TCM symptoms and diseases were searched in Sym Map and HERB database.The drug target was intersected with the disease gene and the Venn map was drawn.Cytoscape3.7.2 was used to construct the network map of the relationship between drug targets and disease genes.The PPI network diagram of the intersecting genes was drawn by STRING,and the core genes were obtained by MCODE modularization.The core genes were enriched by GO and KEGG.Results:Part I: 1.1.Comparison of baseline data: general situation: sex,age,course of disease and scores of TCM syndromes before treatment(degree of abdominal pain,duration,frequency of attack,abdominal distension,food intake,stool,mood,complexion),main symptoms and total scores of TCM syndromes.There was no significant difference between the two groups before treatment(P > 0.05).2.After 2 weeks of treatment,there were significant differences in TCM syndrome scores and total scores in the treatment group(P < 0.05),but there was no significant difference in emotional scores in the control group(P > 0.05).There were significant differences in the other 7 scores and total scores(P < 0.05).There was no significant difference in food intake and emotion between the two groups after 2 weeks of treatment(P >0.05),but there were significant differences in other symptoms,main symptoms and total scores of TCM syndromes between the two groups(P < 0.05).The curative effect of main symptoms and TCM syndromes in the treatment group was better than that in the control group,and the difference was statistically significant(P < 0.05).3.After 4 weeks of treatment,there were significant differences in TCM syndrome scores and total scores between the treatment group and the control group(P < 0.05).There was no significant difference in emotion between the two groups after 4 weeks of treatment(P > 0.05),but there was significant difference in the scores and total scores of the other 7TCM syndromes between the two groups(P < 0.05).After 4 weeks of treatment between the two groups,the efficacy of the main symptoms and TCM syndromes in the treatment group was better than that in the control group,with statistical difference.Safety evaluation: there were no adverse reactions in the two groups,and the clinical application was safe.Part two: 1.33 active components and 201 drug targets were selected from the database.455 disease genes were obtained from Sym Map and HERB database.29 overlapping genes were obtained by the intersection of drug targets and symptom genes,and the PPI network was constructed.14 gene 1 networks were modularized.The biological functions and pathways of genes were obtained by GO and KEGG enrichment analysis through DAVID database,such as negative regulation of cell biosynthesis,immunoglobulin response,cytokine-mediated signal pathway and so on.Cytokine activity,growth factor activity,growth factor receptor binding,cytokine receptor binding and other molecular functions,JAK-STAT signal pathway,NF-k B signal pathway,PI3K-Akt signal pathway,intestinal immune network Ig A production and other pathways.Conclusion: 1.After a comparable study,combined with Yunpi Xiaozhu massage combined with dried seabuckthorn emulsion and simple oral dried seabuckthorn emulsion are effective in improving the degree,frequency,time,abdominal distension,poor appetite,dry stool and complexion of children with abdominal pain,and the curative effect of the treatment group is better than that of the control group.In terms of emotion regulation,there is no difference between the two treatments.2.The method of invigorating the spleen and relieving stagnation and oral administration of dried seabuckthorn emulsion are safe treatment methods.3.Hippophae rhamnoides can act on this syndrome through the signal pathways of inflammation,immunity and nerve system mediated by cytokines and immune proteins,which has something in common with the mechanism of massage.Hippophae rhamnoides and massage can shorten the course of treatment and improve the curative effect,which is worth popularizing. |
