Objective:On the basis of consulting related literature,research on frailty screening of elderly in nursing homes and existing care problems;at the same time,semi-structured interviews are conducted based on grounded theory to analyze the subjective care needs of frail elderly,and then take care issues and care needs as examples.Depending on the basis,construct a care plan for the debilitated elderly in elderly care institutions to provide a reference for the care of the debilitated elderly.Methods:In this study,using the questionnaire survey method,using the frailty phenotype assessment form(FP)and the international long-term care assessment form(inter RAI-LTCF),a total of 416 people in 3 elderly care institutions in Heilongjiang Province were screened for frailty and existing care problems.Using qualitative research,select some frail elderly people to conduct pre-interviews,and adjust and revise the interview outline based on the interview results to form a formal interview outline.Guided by grounded theory,use NVivo11 software to perform three-level coding on the original data to obtain the care needs of the frail elderly.Through expert letter inquiries,the questionnaire survey results are combined with qualitative research results to initially construct a care plan for the frail elderly in elderly care institutions,and conduct two rounds of expert letter inquiries.After each round of letter inquiries,the research team conducts statistical processing of the results Combine expert feedback to sort out and analyze,and from the next round of questionnaires.After the expert opinions reached consensus,the letter of inquiry was completed.Results:1.The incidence of debilitating aging in elderly care institutions is 48%.2.There are 19 types of care problems for frail elderly in elderly care institutions,which are insufficient physical activity,ADL activity limitation,physical restraint,insufficient cognitive ability,delirium,limited communication,depression,abnormal behavior,insufficient social activity,and social Restricted communication,risk of falls,pain problems,risk of pressure ulcers,cardiovascular and respiratory problems,malnutrition,nasal feeding tube problems,insufficient preventive health measures,urinary incontinence,fecal incontinence.3.There are 4 three level indicators(needs for physical health,environmental construction,maintenance of social function,and mental and mental health)for the care needs of frail elderly people,and 13 secondary indicators(daily life care,management of diseases and poor health,management Road management and implant management,physical function rehabilitation,traditional Chinese medicine health care,convenient medical treatment,physical environment,humanistic environment,sense of belonging,social activities,realization of social value and self-dignity,care and conversation,psychological consultation),44 first level indicators(Bath,wash,eat,walk,exercise,coexistence of multiple diseases,pain,two stool control,oxygen therapy,skin care,fistula sterilization,nasal feeding tube care,catheter care,urine bag replacement,heart stent,artificial Joints,poor physical activity,swallowing function,language function,health knowledge,health exercises,traditional Chinese medicine and diet,first aid,hospital green channels,comfort,cleanliness,safety,faith,harmony,harmony,social public opinion creation,family support,desire for partners,Communication,interpersonal communication,recreational activities,participation in daily life management,self-image modification,loneliness,depression,upset,depression,anxiety,fear).4.This study combines the care problems and care needs of the frail elderly,consults the literature,discusses with experts to formulate a care service plan,and applies Delphi method to expert letter inquiries,and finally builds a care service plan for the frail elderly in elderly care institutions.Conclusions:1.The care problems and care needs of the frail elderly in elderly care institutions are characterized by multiple dimensions and diversity.2.Based on the inter RAI-LTCF survey and qualitative research results,a care problem was formed.A care plan was formed through literature surveys and research group discussions.After Delphi method experts’ letter inquiries,a care service plan for the debilitated elderly in elderly care institutions was built to meet The multi-dimensional and diversified care needs of the frail elderly.The program is scientific and reliable,and provides a reference for the care of frail elderly in elderly care institutions.
