Objective To explore the relationship between insulin resistance and abnormal uterine bleeding caused by perimenopausal ovulation disorders;to explore the high-risk factors that affect AUB-O during perimenopause.Methods(1)From September 2019 to October 2020,we selected 107 patients who were diagnosed as perimenopausal AUB-O patients based on medical history, physical signs and auxiliary examinations at the gynecological clinic of the General Hospital of Ningxia Medical University from September 2019 to October 2020.A total of 105 patients with perimenopausal non-AUB-O patients, their general information:(age, pregnancy times,parity,BMI,waist-to-hip ratio, history of past menstrual disorders, occupation, education level, exercise habits, eating habits, whether Hypertension, family history of hypertension, family history of diabetes, family history of diabetes, family history of menstrual disorders)and insulin resistance related index detection:(serum insulin, fasting blood glucose,HOMA-IR,HOMA-β)for comparative analysis;(2)Perimenopausal AUB-0 patients were divided into insulin resistance group and non-insulin resistance group according to HOMA-IR≥1.70,and their general information and insulin-related indicators were compared and analyzed.Results(1)The comparative analysis of the general data of 107 perimenopausal AUB-O patients and 105 perimenopausal non-AUB-O patients showed that the results of BMI,WHR,past menstrual disorder history, exercise habits, eating habits, education level,Diabetes history and other aspects are statistically significant;but there is no statistical difference in occupation, pregnancy times, parity, history of hypertension, family history of hypertension, family history of diabetes, family history of menstrual disorders, etc.;fasting blood glucose in insulin-related indicators The differences in serum insulin,HOMA-IR,HOMA-β,etc.were statistically significant. Then 107 patients with perimenopausal AUB-0 were divided into insulin resistance group with 59 cases and non-insulin resistance group with 48 cases according to HOMA index ≥1.70.The comparative analysis of general data showed that the results showed that the weight,BMI,WHR,and history of hypertension were respectively There are statistically significant differences in terms of age, pregnancy times, parity, occupation, exercise habits, eating habits, education level, past menstrual disorders, menstrual disorders, hypertension, and diabetes. There was no statistically significant difference in other aspects;there were statistically significant differences in insulin-related indicators in fasting blood glucose, serum insulin,HOMA-IR,and HOMA-β.(2)The logistic regression analysis of the influencing factors of AUB-O in perimenopause shows that HOMA-IR,history of menstrual disorders,WHR,serum insulin,HOMA-β,eating habits, and exercise habits are the risks of patients with AUB-O in perimenopause factor. Conclusion There is a significant correlation between insulin resistance and AUB-O in perimenopausal period.Past menstrual disorders,waist-to-hip ratio, poor lifestyle(exercise habits, eating habits),and HOMA index are high risk factors for the onset of AUB-O in perimenopausal period.
