| Objective: To explore the prognostic value of isocitrate dehydrogenase 1(IDH1)and telomerase reverse transcriptase(TERT)promoters in high-grade gliomas and the prognostic value of their integrated analysis.Methods: Collected patients with craniotomy in our hospital from January 2016 to June2020,and postoperative pathological diagnosis of patients with high-grade glioma.Postoperative specimens and blood tests were submitted for examination.IDH1 and IDH1 were examined by second-generation sequencing.For TERT mutations,patients are grouped according to the results of genetic testing,and the impact of different groups on the prognosis of patients is analyzed.Results: Among 62 patients with high-grade glioma,the median survival time of IDH1 mutant and wild-type patients was different(P<0.001).The COX regression model analysis of IDH1 can be used as the prognosis of patients with high-grade glioma.Independent influencing factors;the median survival time of TERT mutant and wild-type patients is different(P<0.05),but TERT cannot be used as an independent factor affecting the prognosis of patients with high-grade glioma by COX regression model analysis;IDH1,TERT mutations The integrated analysis found that there were significant differences in the prognosis of the four groups of patients(P<0.001).The prognosis of the four groups of patients from good to bad was as follows: IDH1 mut/TERT mut>IDH1 mut/TERT wt>IDH1 wt/TERT wt>IDH1 wt/ TERT mut.Conclusion: IDH1 and TERT promoter mutations are important genetic changes in high-grade gliomas.IDH1 mutations are a good prognostic marker.On this basis,the TERT gene mutation status can help further the prognosis of patients with high-grade gliomas.In a stratified study,the prognosis of the four groups of patients from good to bad was as follows: IDH1 mut/TERT mut>IDH1 mut/TERT wt>IDH1 wt/TERT wt>IDH1 wt/TERT mut. |
