| ObjectivesThrough application of Traditional Chinese Medicine(TCM)Combination Therapy on mild to moderate acne vulgaris patients,including TCM mask pack,TCM glanules and health education,this paper aims at observing the overall clinical effects of TCM Combination Therapy on acne patients in improving the physical and mental health situation,and observing the potential adverse effects at the same time,in order to provide standards and formation of programs,which would be suitable for the promotion of medical structure and health care and health care institutions.Methods1.In literature study part,the literatures about treatment of acne with TCM mask pack were retrieved from CBM,CNKI,Wangfang Database and Weipu Database.The publication time of the retrieved literature was from January1,2000 to December 31,2016.By use of the bibliometrics analysis,the publication time,the form of TCM mask pack,the herbal component of TCM mask pack,adverse effects,treatment period,treatment methods and its high frequency,were statistically analyzed.2.In clinical study part,a random clinical trial was performed.First,280 cases with mild to moderate acne vulgaris were enrolled,and were random divided into TCM group(n=140)and control group(n=140),in accordance with cluster random sampling methods.Then,cases in TCM group and control group were treated lasting for 8 weeks,with(TCM)Combination Therapy and external western medicine therapy,separately.At 2weeks,4weeks,6weeks and 8weeks during the period of treatment and at 4weeks after all treatments,score of The Cardiff Acne Disability Index(CADI),Clinical Global Impression(CGI)and Global Acne Grading System(GAGS)were measured repeatedly.In addition,routine urine and blood tests,liver and kidney function test,and electrocardiogram test were performed before and after treatment.By use of SPSS software,repeated measurements anova analysis,analysis covariance,t test and Chi square test were performed.Results1.In literature study part,first of all,a total of 466 papers were published from 2000 to 2016,which reach a peak in 2009.Second,in related literature,the common form of TCM mask pack including reverse mould mask,patch mask,cream,wet compress,sonophoresis,washing agent,herbal fumigation,in order.Third,multiple Chinese herbal were used in mask pack to treat the acne,and the top 10 herbs which are common used includes Dahuang,Huangqin,Danshen,Baizhi,Huangbo,Huanglian,Jinyinhua,Liuhuang,Bingpian,Juhua.Forth,the common treatment period includes 4 weeks,8 weeks,6 weeks,2 weeks,less than 2 weeks,more than 12 weeks,12 weeks,3 weeks,5 weeks,10 weeks,7 weeks,and 9 weeks,in order.The most common treatment period is 4weeks with 193 reported articles,accounting for 41.4 %.Fifth,multiple treatment methods has been applied in related researches.TCM mask patch combined with oral TCM herbs is the most common methods,with 146 reported articles,accounting for 31.33%。In addition,adverse effects were reported in 41 articles,including allergy,dizzy,abdominal pain and diarrhea.The incidence rate varied from 0.1～22%.2.In clinical study part,first of all,a total of 283 cases were enrolled,including 141 cases in TCM group and 142 cases in control group.During the whole process of clinical trial,study was completed on 264 out of the 283 subjects with a loss-to-follow-up rate of 9.22% in TCM group(13 cases)and4.23% in control group(6 cases).Second,at 2 weeks of treatment,GAGS score of patients in TCM group decreased significantly,in comparison with that before treatment(P <0.05).Meanwhile,no significant difference was found in control group(P >0.05).Third,at 4 weeks of treatment,CADI score,SI score and GAGS score of patients in two groups decreased significantly,in comparison with that before treatment(P <0.05).SI and GI score of patients in TCM group was lower than that in control group,significantly(P <0.05).Forth,at 6 weeks of treatment,GAGS scores of patients in both two groups were lower than that at 2 weeks and 4 weeks of treatment and before treatment significantly(P <0.05).Fifth,at 8 weeks of treatment,CADI score,SI score and GAGS score of patients were lower than that before treatment in both two groups significantly(P<0.05).SI score,GI score and GAGS score of patients in TCM groups were lower than that in control group(P <0.05).Sixth,at 4 weeks after treatment,GAGS score of patients in two groups,were lower significantly than that at 8 weeks of treatment(P <0.05),meanwhile no significant difference was found in CADI score,SI score and GI score(P >0.05).GAGS score,CADI score and SI score of patients in two groups were lower than that before treatment,significantly(P <0.05).Seventh,in TCM group,allergy phenomenon occurred in 3 patients,with symptom of erythema.The incidence rate was 2.1%.In control group,allergy phenomenon occurred in 25 patients,with symptom of erythema,dry,pruritus and flaking skin.Eighth,at the end of treatment,no significant abnormity was found in the result of routine urine and blood tests,liver and kidney function test,and electrocardiogram test in both two groups.Conclusion1.In related literatures,the form of TCM mask pack,the herbal component of TCM mask pack,the treatment method and treatment period are various.In detail,reverse mould mask is the most common form of TCM mask pack,meanwhile few research focused on patch mask.In addition,the components are mainly antipyretic and relieving herbal drugs,combined with oral TCM herbs therapy at the same time.2.The random clinic trial proved that both combined TCM mask pack and oral glanules therapy,and external western medicine therapy have satisfying clinic effect on patients with mild to moderate acne,especially in reducing the average scores of skin lesion and improving the mental health.Moreover,combined TCM mask pack and oral glanules therapy may have better clinic effects than external western medicine therapy,with less adverse effects at the same time.In addition,the suitable treatment period is about 8 weeks,and symptoms are not easy to rebound withdraw drug. |
