| Objective To construct the knowledge system of nurses’ suicide prevention training for inpatients,and carry out empirical research to test the effectiveness and practicability of the knowledge system,so as to provide a basis for clinical nurse training.Methods This study adopts descriptive research in non-experimental research and before-and-after control study in quasi-experimental research,combined with a variety of methods such as questionnaire survey,literature analysis,Delphi expert consultation and empirical training.In the first step,literature analysis was used to understand the current situation of inpatient suicide training at home and abroad,and the entry pool of inpatient suicide training knowledge system was constructed,and the training items in the entry pool were compiled into a questionnaire on the knowledge needs of nurses in the prevention of inpatient suicide training.The second step is to conduct a survey on the knowledge needs of inpatient suicide training among 150 nurses from 8 general hospitals in Hubei Province,and to develop a preliminary draft of the knowledge system of inpatient suicide prevention training.The third step is to use the Delphi expert consultation method to conduct expert consultation on the preliminary draft of the training knowledge system of suicide prevention for inpatients,to solicit expert opinions,and to revise the final draft.The fourth step is to carry out empirical training.47 clinical nurses were trained with the constructed knowledge system of inpatient suicide prevention training.Before and after the training,nurses’ self-confidence questionnaire,suicide opinion questionnaire and inpatient suicide nursing management check list were used to conduct questionnaire survey and compare the training effect.Results1.Among the 150 nurses who participated in the survey,48.0% of the nurses had experienced inpatient suicide,and only 59.3% had received inpatient suicide training.The nurses’ demand for suicide prevention training content of patients was > 2,which was between need and very need.The top three requirements of nurses for patient suicide prevention training knowledge were: signs of patient suicide behavior,communication skills of patient suicide risk assessment,intervention of patient suicide ideation/suicide attempt/suicide death;The following three requirements for patient suicide prevention training were cognitive behavioral therapy(CBT),epidemiological and socio-demographic characteristics of patient suicide,and theory of patient suicide.2.A total of 25 consulting experts were included in this study,including 24 experts who responded to the first round,with a response rate of 96.0%.Among them,17 experts put forward suggestions for revision,accounting for 70.8%;In the second round,24 questionnaires were sent out and 22 were returned,with a response rate of91.7%.Among them,8 experts put forward suggestions for revision,accounting for36.4%.The authority coefficient,judgment coefficient and familiarity coefficient of the two rounds of expert consultation were 0.88,0.87 and 0.88 respectively.0.83,0.85,0.81.The Kendall coefficient of harmony of the two rounds were 0.29 and 0.17,respectively,the differences were statistically significant(χ2 =152.15,χ2 =77.26,all P <0.001).3.After two rounds of expert consultation,6 items were deleted,7 items were added and 4 second-level items were modified.The knowledge system of suicide training for inpatients finally formed included 4 first-level items and 26 training items.4.Before the training,the scores of 47 nurses’ self-confidence questionnaire,suicide opinion questionnaire and inpatient suicide nursing management checklist were12.02±3.27,161.28±15.63 and 17.19±4.98,respectively.After inpatient suicide prevention knowledge system training,the scores of the three questionnaires were16.49±0.35,194.74±24.71 and 22.26±1.75,respectively,the differences were statistically significant(t=-8.58,t=-7.07,t=6.64,all P < 0.001).Conclusions1.Nurses lack confidence in suicide prevention of inpatients,and their overall attitude and cognition are at a medium level.But nurses have the consciousness of preventing inpatient suicide,the basic measures of preventing inpatient suicide have been basically implemented.2.The rate of nurses receiving inpatient suicide prevention training is relatively low,and the relevant theoretical knowledge is insufficient,so there is a high demand for inpatient suicide prevention training knowledge.3.Guided by nurses’ needs,the knowledge system of inpatient suicide training constructed by Delphi expert consultation method contains four first-level indicators:basic knowledge of patient suicide,suicide risk assessment,patient suicide prevention,and referral of high-risk patients to suicide,a total of 26 training projects.The training knowledge system is proved to be practical and effective.4.To carry out suicide prevention training for nurses in inpatients can effectively improve the confidence,attitude and cognition level of nurses in preventing suicide in patients,and effectively promote the implementation of suicide measures in inpatients. |
