Objective:Based on the Carolina care model,the training program of humanistic care ability of nurses was constructed to verify the application effect of the five core behaviors of Carolina care model in obstetrics and gynecology nurses,and to improve the quality of clinical nursing service.Methods:1.A Baseline Survey of Nurses’Humanistic Care Ability(1)a cluster sampling method was used to investigate the nurses of obstetrics and gynecology department in a tertiary general hospital in Chongqing,using the basic information questionnaire and the caring Ability Inventory(the caring Ability Inventory,CAI)questionnaire to understand the status of humanistic care ability and analyze its influencing factors.(2)SPSS 24.0 was used to analyze the data,and the basic information of nurses was expressed in terms of work frequency and proportion.The evaluation of nurses’humanistic care ability was calculated by mean±standard deviation.Independent sample t-test and one-way ANOVA were used to analyze the influencing factors of humanistic care ability of nurses in obstetrics and gynecology department.The significance level a=0.05,P<0.05 was considered to be statistically2.Construction of training program of humanistic care ability for nurses in obstetrics and gynecology department(1)Based on the investigation of the status quo,through the retrieval of relevant literature,according to the core behavior of Carolina care model,combined with the characteristics of holistic nursing model,the training content of humanistic care for nurses in obstetrics and gynecology was initially determined.(2)Through brainstorming,inviting obstetrics and gynecology nursing experts,humanistic nursing research experts and senior nursing responsibility team leaders to discuss the training content,and finally form the humanistic care training program for obstetrics and gynecology nurses.3.Evaluation on the training effect of nurses’humanistic care ability(1)a total of 40 nurses from a tertiary general hospital in Chongqing were randomly divided into intervention group(n=20)and control group(n=20);Forty patients in the intervention group were divided into intervention group a(n=40),and 40 patients in the control group were divided into control group B(n=40).(2)The intervention group was trained with humanistic care program based on Carolina care mode,while the control group was trained with holistic nursing mode and humanistic care content(such as health education,communication,psychological nursing,etc.).(3)The evaluation index included three aspects:(1)Nurses’humanistic care ability,including three dimensions:cognition,courage and patience;(2)Perception of care;(3)Satisfaction of patients.SPSS 24.0 was used to analyze the data,P<0.05 was considered statistically significant.The differences of general demographic data of nurses and patients between the two groups were compared by?~2test;(2)The changes of nurses’humanistic care ability,patient care perception score and patient satisfaction score before and after humanistic care training were compared by t test,and the intervention effect after humanistic care training was analyzed.Results:1.Baseline survey resultThe scores of each dimension of nurses’humanistic care ability from high to low were courage dimension(68.80±8.64),cognitive dimension(54.63±6.75),patience dimension(45.22±7.15),and total demand score(168.65±17.35).Univariate analysis showed that the way of employment and professional title were the important social factors affecting the score of humanistic care ability of nurses(P<0.05).There were 30 nurses with low humanistic care ability,accounting for 75%,8 nurses with medium humanistic care ability,accounting for 20%,and 2 nurses with strong humanistic care ability.Nurses’humanistic care ability scores from high to low were courage dimension(68.80±8.64),cognitive dimension(54.63±6.75),patience dimension(45.22±7.15),and total demand score(168.65±17.35).Univariate analysis showed that the way of employment and professional title were the influencing factors of humanistic care ability(P<0.05).There were 30 nurses with low humanistic care ability,8 nurses with medium humanistic care ability and 2 nurses with strong humanistic care ability.2.Formation of training programmesThe training program of humanistic care for nurses based on Carolina care model was formed,which mainly consisted of four aspects:(1)Theoretical knowledge:collective teaching was adopted,and the training was completed in four weeks by combining online and offline teaching during the epidemic period;(2)Perceptual care:Participate in the activities of Nurses’Warmth Month,watch classic humanistic movies,scene simulation and role play in groups,and the training should be completed within one month;(3)Skill practice:It is mainly based on the five core behaviors of Carolina care model,including five parts:a.Service and etiquette norms based on humanistic care;b.Carry out round structure multilevel ward round every day by combining care proces elements;c.3to 5 minutes a day to communicate with patients;d.Refining the overall responsibility system of nursing service process;e.Combining with the characteristics of gynecology and obstetrics specialty,the ISBAR shift template was formulated to enable patients to participate in bedside shift,which was practiced in the first two weeks of each month and maintained in the last two weeks;(4)Caring experience:Share your personal experience and feelings in the form of group discussion.3.Training effect(1)The scores of nurses’humanistic care ability:The scores of nurses’humanistic care ability in cognition,courage and patience and the total scores in the control group increased from 53.30±5.27,68.55±5.66,44.40±4.58 and 166.25±11.10 before the training to 54.25±8.23 and69.90 after the training.7.08 points,44.45±5.83 points,168.39±13.26points.There was no significant difference in the scores of humanistic care ability of nurses in the control group before and after training(P>0.05);The scores of cognition,courage,patience and total scores of humanistic care ability of nurses in the intervention group increased from55.95±6.02,69.05±7.02,46.05±6.32 and 171.05±15.27 before training to 59.6±5.92,73.85±5.30 and 48.20±4.3 after training 2 points,181.65±9.78 points,the total score of humanistic care ability and the scores of cognition,courage and patience of nurses in the intervention group before and after training were significantly higher than those before training(P<0.05).(2)The total score of care perception of patients in group B increased from 51.45±5.36 to 51.93±5.74 before and after training.There was no significant difference in care perception of patients in group B before and after training(P>0.05);Before and after training,the total score of care perception of patients in intervention group a increased from 51.65±3.87to 54.33±2.41.Except the items that patients felt respected by nurses and that nurses made patients feel comfortable,the other items showed significant differences in care perception of patients(*P<0.05,**P<0.01).(3)Patients’satisfaction:The total score of patients’satisfaction in group B increased from 139.23±14.95 to 139.63±11.98,there was no significant difference(P>0.05).The total score of patients’satisfaction in intervention group a increased from 138.45±16.57 to 147.85±7.18before and after training,and the total evaluation of patients’satisfaction in intervention group a was higher than that in control group B after training,with significant difference(P<0.05).Conclusions:1.The humanistic care ability of nurses in obstetrics and gynecology is generally at a low level,so it is necessary to carry out the training of humanistic care ability.2.Nurses’humanistic care ability was related to their professional titles and ways of employment.Nurses with higher professional titles had higher humanistic care ability;The ability of humanistic care of nurses who are employed on the staff is higher than that of contract nurses.3.Based on Carolina care model training,the program consists of theoretical knowledge,perception of care,skills practice(based on Carolina care model five core behavior),care experience four parts,after training can effectively improve the ability of humanistic care of nurses in obstetrics and gynecology,enhance patient care perception,improve patient satisfaction,worthy of clinical promotion.
