| Objective:In female gential tract malignant tumors, the incidence of cervical cancer is at the top of the list, which is a great threat to the health of women. At present, the scholars believe that persistent human papilloma virus infection is the main cause of cervical intraepithelial neoplasia and cervical cancer. The main purpose of this paper is to investigate the type distribution of human papillomavirus infection and its correlation with cervical intraepithelial neoplasia and cervical cancer in the Central Hospital of Taian. The prevention of infection provides some data support, and provides basic reference for the screening, diagnosis and treatment of cervical lesions, and provides some data support for vaccination against cervical cancer. Methods:Patients were collected from the cervical disease clinic of Tai’an City Center Hospital from January 2017 to October 2018.Patients voluntarily performed cervical cytology screening(TCT)screening and HPV infection screening and cervical biopsy,HPV positive patients were enrolled,Patients were divided into chronic cervicitis group, the low grade squamous intraepithelial lesion(LSIL),high-grade squamous intraepithelial lesions(HSIL)and cervical cancer(CA)group based on cervical biopsy. We researched the distribution of HPV subtypes among the four groups, and compared the differences of HPV infection in different degrees of cervical lesions.Result:1.Among 9120 women, the HPV infection rates were 25.66%(2340/9120),the top ten HPV types were HPV16(4.24%),HPV52(3.68%),HPV58(3.42%),HPV51(2.36%),HPV39(2.34%),HPV56(2.03%),HPV66(1.62%),HPV18(1.20%),HPV59(0.98%),HPV35(0.84%).2.According to the result of TCT,there were 512 patients who underwent colposcopy cervical biopsy, there were 396 cases of cervical lesions.Among 512 patients, the top 12 HPV infection subtypes were(decreasing)HPV16(48.43%),HPV58(8.40%),HPV18(7.03%),HPV52(6.48%),HPV33(2.93%),HPV66(2.34%),HPV51(2.15%),HPV56(1.95%),HPV39(1.76%),HPV35(1.56%),HPV31(1.17%),HPV53(0.98%).The common HPV types in chronic cervicitis are: HPV16(34.48%),HPV58(10.34%),HPV18(7.76%),HPV52(7.76%),HPV51(6.90%),HPV56(5.17%),HPV39(3.45).%),HPV33(2.59%),HPV66(2.59%),HPV53(1.72%);The common HPV types in low-grade squamous intraepithelial lesions are: HPV16(27.27%),HPV18(11.11%),HPV58(9.09%),HPV52(8.08%),HPV66(8.08%),HPV35(3.03%),HPV39(3.03%),HPV51(3.03%),HPV53(3.03%)and HPV56(2.02%);The common HPV types in high-grade squamous intraepithelial lesions are: HPV16(58.59%),HPV58(8.59%),HPV52(7.03%),HPV18(3.91%),HPV33(3.91%),HPV31(1.56%),HPV35(1.56%),HPV39(0.78%),HPV56(0.78%)and HPV6(0.39%);The common HPV types in cervical cancer were HPV16(75.61%),HPV18(14.63%),HPV33(2.44%)and HPV82(2.44%);2.The peak age of HPV infection in the collected patients was 30-49 years old, with HPV16 infection being the most common subtype. The 30-49 years old is common in chronic cervicitis,low-grade squamous intraepithelial lesions,high-grade squamous intraepithelial lesions, and cervical cancer.but with the severity of the disease, the proportion of this age group shows a downward trend, and the proportion of the age of 50 years old and above is on the rise;3.The proportion of multiple HPV-infected subtypes gradually decreases as the level of cervical lesions increases;4.HPV16 has a high proportion in chronic cervicitis, cervical lesions and cervical cancer.Conclusion1.HPV16 subtype infection is the most common among cervical lesions, followed by HPV58 subtype,HPV52 subtype,HPV18 subtype and HPV33 subtype;the first three HPV infections showed regional differences compared with other regions, and no cases of HPV45 subtype infection were found.2.In the cervical lesions group of different grades, the proportion of HPV16 subtype increased with the increase of cervical lesions, even up to 75.61% in the cervical cancer group;3.There was no correlation between multiple HPV infections and cervical lesions. |
