Objective:Investigating and studying the 5-year epidemiological characteristics of hip fractures in the elderly in Changsha to provide data and theoretical basis for active prevention and treatment of hip fractures in the elderly.Methods: The database of Changsha City Medical Security Bureau was searched for information on the first page of cases discharged with the diagnosis of intertrochanteric fracture,intertrochanteric fracture and femoral neck fracture between 1 January 2014 and 31 December 2018,and a total of 9706 cases were retrieved,of which 159 cases had duplicate information due to midway transfer The remaining 9547 cases were retrospectively analysed for age,gender,fracture type,number of days in hospital,cost of built-in consumables and total inpatient medical costs.Result:(1)The number of elderly hip fractures in 2014-2018 was 542,1199,2266,2566 and 3014,with crude incidence rates of 7.41/100000,29.12/100000,29.12/100000,32.41/100000 and 36.96/100000,respectively.(2)There were 3042 elderly male hip fractures(31.9%)and6505 elderly female hip fractures(68.1%).(3) Based on the age of the patients at the time of admission,the statistics for each age group are as follows: 2303(24.1%)aged 60-64,974(10.2%)aged 65-69,1123(11.8%)aged 70-74,1640(17.2%)aged 75-79 and 3507(36.7%)aged 80+.(4)The mean age of admission was 75.07±11.92 years for female elderly hip fracture patients and 68.86±16.31 years for male elderly hip patients.(5)The number of elderly hip fractures in Changsha from January to December 2014-2018 was 862,711,712,664,663,648,781,788,718,921,937,1142 and the proportion of patients in each month was 9.03%,7.45%,7.46%,6.96%,6.94%,6.94%,respectively 6.79%,8.18%,8.25%,7.52%,9.65%,9.81%,11.96%.(6)A total of 6197 patients were treated surgically,with an average length of stay of 18.83±10.89 days,an average cost of built-in consumables of RMB 17572.12±11816.79 and an average total medical cost of RMB 40018.80±24240.69.(7)The number of patients with femoral neck fractures was 8540 and the number of patients with intertrochanteric fractures was 1007,a ratio of 8.48:1.(8)The number of elderly male patients with femoral neck fractures and intertrochanteric fractures was 3651 and 391 respectively,a ratio of 9.34:1,while the number of elderly female patients with femoral neck fractures and intertrochanteric fractures was 5889 and 616 respectively,a ratio of 9.56:1.Conclusion: 1.The incidence of crude hip fractures in the elderly is increasing year by year.2.Geriatric hip fractures account for the highest percentage of people over 80 years of age and the second highest percentage of patients in the 60-64 age group.3.There were more female elderly hip patients than male,a higher percentage of patients in winter and spring than in summer and fall,and more femoral neck fractures than intertrochanteric fractures.4.The average length of stay,cost of built-in consumables(hip fracture),and total medical costs for surgical patients have been decreasing year by year.
