Objective:This clinical study used Xuanbai Chengqi Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease(lung heat Fu syndrome),to evaluate the clinical efficacy and safety of Xuanbai Chengqi Decoction in the treatment of AECOPD(lung heat Fu syndrome). Methods:In this study,72 patients with AECOPD(lung heat and viscera syndrome)were randomly divided into treatment group and control group,36 cases in each group. The control group was given conventional western medicine treatment, and the treatment group was given conventional western medicine treatment combined with Xuanbai Chengqi Decoction. The course of treatment was 7 days. The general information, efficacy and safety indexes of the patients were recorded, and the data were processed and analyzed by statistical software spss26.0.Result:1.Baseline comparability: there was no significant difference in gender, age, course of disease, smoking history, pulmonary function classification between the two groups(P > 0.05)2.Comprehensive clinical efficacy of traditional Chinese medicine: after treatment, both groups achieved a certain effect, the effective rate of the treatment group was 94.44%,the effective rate of the control group was 88.89%,the difference between the two groups was statistically significant(P < 0.05),and the effect of the treatment group was better than that of the control group. The total score of TCM clinical efficacy, single TCM syndrome score, cat questionnaire and MMRC score: after treatment, the four scores of the two groups were lower than before treatment(P < 0.05),and the improvement of the treatment group was better than that of the control group, the difference was statistically significant(P < 0.05).4.Lung function,6-minute walk test, blood gas analysis: after treatment, the indexes of 6-minute walk test, blood gas analysis(PaCO2,PaO2,Sa O2)and lung function(FEV1,FVC,FEV1 / FVC)of the two groups were improved compared with those before treatment(P < 0.05).After treatment, the improvement of pulmonary function(FEV1,FVC),6-minute walk test and blood gas analysis indexes in the treatment group were better than those in the control group, the difference was statistically significant(P <0.05);the difference of pulmonary function(FEV1 / FVC)between the two groups was not statistically significant(P > 0.05).5.Peripheral blood leukocyte count(WBC),neutrophil percentage(n%),C-reactive protein(CRP): after treatment, the WBC,n%,CRP values of the two groups were significantly improved compared with those before treatment(P < 0.01),and the improvement of the treatment group was significantly better than that of the control group(P < 0.01).6.Safety index: before and after treatment, the safety indexes of alanine aminotransferase(ALT),aspartate aminotransferase(AST),urea(BUN),creatinine(CR),platelet(PLT)in the two groups were not significantly abnormal, the difference was not statistically significant(P > 0.05),no drug-related adverse reactions occurred, and the safety evaluation was grade 1.Conclusion:Xuanbai Chengqi Decoction Combined with conventional western medicine in the treatment of AECOPD(lung heat Fu syndrome)is more effective than conventional western medicine alone. It can better improve the symptoms and signs, inflammatory indicators, lung function and other indicators of patients, improve the quality of life of patients, and the prescription is safe without adverse reactions. It provides clinical evidence for the treatment of AECOPD with traditional Chinese medicine.
