Objective: Observing the vessel skeleton density(VSD)and vessel area density(VAD)in the macular area of primary open-angle glaucoma(POAG)and studying the effect of yimaikang tablets on macular area vascular density in primary open-angle glaucoma.Methods:In the first part,42 POAG patients(75 eyes)and 20 healthy adults(40 eyes)without any ocular or systemic diseases were recruited.BCVA(best corrected visual acuity),IOP(intraocular pressure),VSD and VAD in the macular area,GCIPLT(ganglion cell inner plexiform layer thickness),MRT(macular retinal thickness),MD(mean deviation)were measured firstly,and the correlationbetween VSD,VAD and GCIPLT,MRT,MDwere analyzed to evaluate the blood flow characteristics of primary open-angle glaucoma.In the second part,the POAG patients whose intraocular pressure is controlled at 18 mm Hg and below were distributed into experimental group and control group randomly.The control group(21 cases,38 eyes)uses basic eye drops only and the experimental group(21 cases,37 eyes)usesbothyimaikang tablets and basic eye drops.Then the MD,VSD and VAD in macular area,GCIPLT,MRT were comparedbefore and after treatment between the two groups.Results:(1)The visual acuity,MD,macular VSD,macular VAD,macular GCIPLT and MRT in POAG group were lower than those in control group,the differences were statistically significant(P<0.05).(2)Correlation analysis results in the POAG group showed that the central,inner,outer,intact VSD and VAD of the macular area were positively correlated with MD and MRT(all P<0.05),with the highest correlation in the outer layer.The outer layer,intact VSD and outer layer,intact VAD of macular region were positively correlated with GCIPLT of macular region(P<0.05).(3)BCVA: There was no significant difference between the control group before and after treatment(P>0.05).The experimental group after treatment was better than before(P<0.05).After treatment,the experimental group was better than the control group(P<0.05).(4)MD: MD was improved in experimental group after treatment,and the difference was statistically significant(P<0.05).After treatment,the experimental group was better than the control group statistically(P<0.05).(5)VSD and VAD in macular area: The outer layer,inner layer,intact VSD and VAD were increased in experimental group before and after treatment,and the differences were statistically significant(P<0.05).There were no significant differences in VSD and VAD in the control group before and after treatment(P > 0.05).There was no significant difference in central VSD and VAD between the two groups after treatment(P > 0.05).After treatment,the outer layer,inner layer,intact VSD and VAD in experimental group were higher than those in control group,and the differences were statistically significant(P<0.05).(6)GCIPLT in macular area and MRT : there were no significant change between the two groups after treatment(P > 0.05).Conclusion:(1)The BCVA,MD,macular VSD,macular VAD,macular GCIPLT and MRT in POAG are lower than healthy adults.(2)The vascular density of each layer in macular area was closely related to MD and MRT.(3)The visual acuity,MD and vessel density can be improved by yimaikang tablets in POAG patients.
