Effect Of Modified Suanzaoren Decoction On Myocardial Remodeling In Patients With Hypertension(hyperactivity Of Fire Due To Yin Deficiency)and Anxiety Disorder

Objective:To explore the effect of modified suanzaoren decoction combined with western medicine on myocardial remodeling in Patients with hypertension(Yin deficiency and fire flourishing type)complicated with anxiety disorder,and to provide a safer and more effective means of integrated traditional Chinese and western medicine for clinical treatment of the disease.Methods:Selected 92 patients hospitalized in cardiovascular from August 2019 to July 2020 in Sichuan Provincial Hospital of Traditional Chinese Medicine and met the conditions of each group as test subjects.They were randomly divided into two groups:treatment group(46 cases were treated with modified suanzaoren decoction combined with standard western medicine)and control group(46 cases were treated with standard western medicine).The treatment course was 24 weeks.Records before and after treatment in patients with blood pressure,Hamilton anxiety scale(HAMA)score,Renin-angiotensin-aldosterone(RAAS),echocardiography,the change of TCM syndrome integral,to evaluate the effect of modified suanzaoren decoction combined with standardized western medicine on blood pressure,myocardial remodeling and symptom improvement of subjects.Results:In this study,a total of 92 subjects were observed,10 of them were removed or removed,and 82 of them were finally completed,including 42 in the treatment group and 40 in the control group.After 24 weeks of treatment,the statistical results showed:1.The effect of hypotension: Both groups could lower blood pressure,and the effect of treatment group was better than that of control group(P < 0.05).2.RAAS indicators: Angiotensin II(Ang II),Aldosterone(ALD)and Plasma renin activity(PRA)could be decreased in both groups,with statistical differences(P< 0.05).After treatment,there were statistically significant differences in Ang II and ALD between groups(P < 0.05),but no statistically significant differences in PRA(P > 0.05).3.Echocardiography:The Left atrial(LA),the Left ventricle Posterior wall thickness(LVPWT),the intervencicular septal thickness(IVST)and the Left ventricular end diastolic diameter(LVET)were reduced in both groups.LVEDD),Left ventricular mass index(LVMI),and there were statistical differences in the group(P < 0.05).After treatment,there were statistically significant differences in LA,LVMI and LVEDD between groups(P < 0.05),while there were no statistically significant differences in IVST and LVPWT(P > 0.05).4.TCM syndrome score: Both groups could improve TCM syndromes,and the treatment group was better than the control group(P < 0.05).5.The effect of anxiety: Both groups could decrease the HAMA scale score,and the treatment group was better than the control group(P < 0.05).6.Safety evaluation: During the study period,there were a total of 3 cases of adverse events,all of which were mild,including 2 cases in the treatment group and 1case in the control group,with no statistical difference in the number of cases(P >0.05).Conclusion : For patients with Yin-deficiency and fire-strength hypertension with anxiety disorder,compared with western medicine alone,the combined treatment of modified suanzaoren decoction can more effectively reduce blood pressure,inhibit myocardial remodeling,relieve patients’ anxiety,and improve patients’ TCM syndromes.Moreover,the research results show that the modified suanzaoren decoction has high safety and has promotion value.
