Background Coronary atherosclerotic heart disease(CAD)is one of the most common atherosclerotic diseases.In particular,with the development of society and economy,people’s diet structure,eating habits and work and rest patterns have changed.As a result,the incidence of coronary heart disease keeps rising and the trend of younger people gradually appears,which poses a great threat to people’s health and makes the social economy bear a heavy burden.Apolipoprotein E(Apo E)gene polymorphism is considered to be one of the important factors in the pathogenesis of coronary heart disease.It is known that Apo E is a 34 k Da serum glycoprotein containing299 amino acids,which is an important component of various lipoprotein particles(chylomicron,very low density lipoprotein,low density lipoprotein).Plays a key role in lipid metabolism.At present,the correlation between them has not been fully clarified.If the association between them and cardiovascular diseases can be revealed and clarified from the gene level,and the specific mechanism of metabolic abnormalities can be clarified,and reasonable regulation methods can be adopted,it will bring value to scientific research,clinical work and the majority of patients.Objective To investigate the distribution of APOE polymorphism and its correlation with coronary artery disease(CAD),blood lipid profile and other cardiometabolic risk factors in Anhui some Han nationality.Methods A total of 199 patients with coronary angiography who were hospitalized in the Department of Cardiology,Hefei Second People’s Hospital from September 2019 to September 2020 were collected.According to different study contents,two groups were adopted(coronary heart disease group and control group,respectively,ε2,ε3 and ε4group).The ε2,ε3 and ε4 alleles and rs7412 and RS429358 genotypes were detected by PCR-fluorescence probe.By comparing the characteristics of coronary artery disease inε2,ε3 and ε4 allele carriers,the relationship between Apo E genotype/subtype and the occurrence and development of coronary heart disease was analyzed.Results 1.The mean age of CAD group(66.0±12.0)years was higher than that of control group(61.2±10.7)years(P =0.005).The mean fasting blood glucose level of control group(5.9±1.3)mmol/L was lower than that of CAD group(7.0±3.8)mmol/L(P=0.019).The average low density lipoprotein C level in CAD group(2.8±0.9)mmol/L was higher than that in control group(2.5±0.6)mmol/L(P =0.038);The levels of C-reactive protein,WBC and cystatin C in CAD group were higher than those in control group,the differences were statistically significant(P =0.014,P =0.010,P =0.026).The two groups in gender,body mass index,smoking and drinking history,history of hypertension and diabetes mellitus,systolic blood pressure and diastolic blood pressure,total cholesterol(TC),average level of HDL((HDL-C),triglycerides(TG),homocysteine levels and apolipoprotein B(Apo B)concentration difference is no statistical significance(p > 0.05).2.The frequency of ε2,ε3 and ε4 in the whole sample was 13.6%,69.3% and 17.1%,respectively,which met the hardy-Weinberg genetic balance and represented the population.The proportion of ε2(20.8%)in the control group was higher than that of ε4(9.4%)(P =0.024),and the distribution frequency of ε4(21.3%)in the CAD group was higher than that of ε2(11.1%)(P =0.038).The proportion of ε3 in the control group and CAD group was 68.1% and 69.3%,respectively.There was no statistical difference(P=0.982);Apo E gene polymorphism was significantly correlated with the incidence of coronary heart disease(ε2: r=0.160,P < 0.05;ε4: r=0.147,P < 0.05);Logistic regression analysis showed that there was no significant correlation between traditional risk factors and the incidence of CHD(P >0.05).3.The analysis of the relationship between Apo E gene polymorphism and blood lipid level showed that there was no significant difference in the distribution of LDL cholesterol,total cholesterol and triglyceride levels in each subtype of Apo E(P >0.05),while the distribution of HIGH-DENSITY lipoprotein level was different in each group,the difference was statistically significant(P <0.05).In addition,among the traditional risk factors of coronary heart disease,the distribution of average BMI level in Apo E subtypes had statistical significance(P <0.05).4.ε2,ε3 and ε4 were 9.4%,69.3% and 21.3%,respectively.There were significant differences in gensini scores between ε2,ε3 and ε4 groups in CAD group(P =0.026,P=0.013,P =0.001),and no significant differences between the degree of vascular stenosis and the number of lesions and Apo E gene polymorphism(ε2=0.15,ε3=0.822,ε4=0.276;ε2=0.324,ε3=0.849,ε4=0.233);Among the criminal vascular lesions,ε4carriers mainly had localized lesions(P=0.027).Conclision The occurrence of coronary heart disease in Anhui some Han nationality is well correlated with Apo E gene polymorphism,which is consistent with the current mainstream research results in this field,and Apo E gene polymorphism is correlated with the distribution of subjects’ high-density lipoprotein and average BMI level,but not with the traditional risk factors of coronary heart disease.The severity of coronary artery disease(Gensini score)and the type of criminal vascular disease are correlated with Apo E gene polymorphism among some han nationality with coronary heart disease in Anhui,which is relatively rare in domestic research results in this field.
