| BackgroundHepatocellular carcinoma,as a malignant tumor of digestive tract with high incidence,seriously threatens human life and health.Liver cancer resection can significantly improve the survival rate of patients with early-stage liver cancer,but the burden of physical and psychological symptoms in patients after surgery is heavy,and the exercise tolerance decreases,which has a negative impact on their rehabilitation.Baduanjin is a traditional exercise in China that can enhance the motor function of patients with breast cancer,lung cancer and other cancers,improve discomfort symptoms,and then improve the quality of life.However,how to construct an accurate Baduanjin training program suitable for patients with liver cancer resection still needs to be explore.Therefore,based on the theory of simultaneous treatment of mind and body and symptom management theory,this study constructed a training program of Wenwu Baduanjin for patients undergoing liver cancer resection,and evaluated its application effect.ObjectiveTo construct a Wenwu Baduanjin training program for patients with liver cancer resection,and to evaluate the application effect of the program,in order to reduce the patient’s symptom burden,improve exercise tolerance and quality of life,provide a practical basis for the formulation of postoperative exercise prescriptions for liver cancer patients,and also provide a basis for Baduanjin.Jin provides a reference for the clinical application of brocade in cancer patients.Methods1.Based on the theory of physical and mental co-treatment and symptom management,literature research,group discussion,and expert correspondence were used to construct the Wenwu Baduanjin training program for patients with liver cancer resection.2.A total of 70 patients with primary liver cancer who met the inclusion criteria were selected as the research objects,and randomly divided into groups by software coding,with 35 cases in the control group and 35 in the intervention group.The control group received routine nursing,and the intervention group received Wenwu Baduanjin training on this basis.The scores of 6-min walking distance,symptoms,anxiety,depression and quality of life were compared between the two groups before the intervention,8 weeks and16 weeks after the operation.SPSS26.0 software was used to conduct t test,test,repeated measures analysis of variance,LSD-t test for the data.Results1.Scheme construction results From the 4th day after operation to 1 week after operation,perform Ba Duan Jin,15 minutes/time,1 time/day;in the second week after operation,perform knee bathing,bathing Legs,Wu Shi Baduanjin No.4,15 minutes/time,2 times/day;bathing legs in the 3rd week after surgery,Wu Shi Baduanjin No.4,7,8,15minutes/time,2 times/day;4 weeks to 8 weeks after surgery,a full set of Ba Duan Jin,15min-20min/time,2 times/day;9 weeks to 16 weeks after surgery,a full set of Ba Duan Jin,20 minutes/time,2 times/day day.The exercise intensity was 40-60%of the maximum heart rate from the 4th day to 3 weeks after the operation,and 60%to 80%of the maximum heart rate from the 4th to 16th week after the operation.2.Comparison of 6-minute walking distance between two groups of patients At 8and 16 weeks after operation,the 6-minute walking distance between the intervention group and the control group was significantly different(t8week=2.086,t16week=3.855,P<0.05).The time effect,inter-group effect and interaction effect of 6-minute walking distance between the two groups were all statistically significant(Ftime=59.716,Fbetween-group=4.442,Ftime*group=40.962,P<0.05).3.Comparison of symptom scores between two groups of patients At 8 weeks after operation,the symptom scores of the intervention group were compared with those of the control group,and the difference was not statistically significant(t8week=-1.722,P>0.05).At 16 weeks after operation,the symptom scores of the intervention group were compared with the control group,The difference was statistically significant(t16weeks=-3.255,P<0.05).The time effect,inter-group effect and interaction effect of symptom scores in the two groups were all statistically significant(Ftime=4.763,F between-group=5.567,Ftime*group=6.520,P<0.05).4.Comparison of anxiety scores between the two groups of patients At 8 and 16weeks after operation,the anxiety and depression scores of the intervention group were significantly different from those of the control group(t8week=-2.439,t16week=-3.594,P<0.05).The time effect,inter-group effect and interaction effect of anxiety scores in the two groups were all statistically significant(Ftime=23.553,Fbetween-group=5.038,Ftime*group=6.689,P<0.05).5.Comparison of depression scores between two groups of patients At 8 and 16weeks after the operation,the depression scores of the intervention group were significantly different from those of the control group(t8week=-2.498,t16week=-6.176,P<0.05).The time effect,inter-group effect and interaction effect of depression scores in the two groups were all statistically significant(Ftime=38.114,Fbetween-group=8.296,Ftime*group=13.659,P<0.05).6.Comparison of quality of life scores between two groups of patients Eight weeks after operation,the emotional status(t8week=0.006)and total score(t8week=2.304)of patients in the intervention group were significantly different from those in the control group(P<0.05).Intervention at 16 weeks after operation Physiological status(t16week=3.844),emotional status(t16week=3.897),functional status(t16week=4.225),liver cancer-specific symptoms(t16week=3.220)and total score(t16week=4.686)in the quality of life of patients in the group compared with the control group,the difference was statistically significant(P<0.05),and the family status at 8 and 16 weeks after the operation was not significantly different from the control group(t8week=1.483,t16week=1.942,P>0.05).Physiological status(Ftime=9.370,Fbetween-group=6.414,Ftime*group=6.188),emotional status(Ftime=22.949,Fbetween-group=7.314,Ftime*group=7.551),functional status(FTime=9.988,Fbetween-group=4.641,Ftime*group=7.317),liver cancer-specific symptoms(Ftime=8.787,Fbetween-group=4.323,Ftime*group=5.387)score and total score(Ftime=23.672,Fbetween-group=8.850,Ftime*group=22.280),the time effect,between-group effect and interaction effect were all statistically significant(P<0.05),but the time effect,between-group effect and interaction effect of family status score were not statistically significant(P>0.05).ConclusionsAccording to the symptom management needs and exercise tolerance of patients undergoing liver cancer resection at each stage,the Wenwu Baduanjin training program constructed through literature research and expert correspondence is highly maneuverable,highly acceptable to patients,and can reduce the patient’s physical condition,symptom burden,enhance exercise endurance,improve anxiety,depression,and improve quality of life. |
